This article reflects something I’ve been talking about for a while. When a diagnosis is used as a way to decide who gets access to resources or not, then this is going to result in inequality. That’s because those who can afford to pay for a diagnosis and those who understand the system will get their diagnoses more quickly. This will then enable them to access public services more quickly. Those who cannot afford to pay or who do not understand the system will be left behind on a long waiting list, or maybe won’t even get that far. There is no blame attached to this observation. It’s not an attack. Of course parents will do whatever they can to help their children, and if they have the resources to pay privately many of them will do that. But if we have a system where public services are dependent on a diagnosis, and that diagnosis can be obtained more quickly if you pay, then we are going to end up with the most affluent people receiving most services. This is a side effect of the burgeoning market in private diagnostic assessments which has sprung up over the last fifteen years. As a society, we need to look clearly at what is happening and decide whether this is the best way to allocate public resources. theguardian.com/society/2026…

I’ve just read a Forbes magazine interview in which PDA, or pathological demand avoidance, is defined by an advocate as a ‘neurobiological command’. People with PDA, she said, have no choice but to respond with avoidance. I then saw a video where parents were told their child with PDA has a fragile nervous system, meaning they inevitably will respond to every day life as if it is a threat. And I heard a podcast, saying that PDA is so serious than children could end up fed by a tube, because eating is a demand and they are wired to avoid demands. All by well known advocates and consultants with hundreds of thousands of followers. Beliefs like this have consequences. They change the way people live their lives. They closes things down, and it shrinks their world. They can cause real harm, as children get stuck in their bedrooms and the whole family organises themselves around avoidance. Yet that is what many well known advocates tell parents whose children are identified as having PDA. Despite the complete lack of evidence, despite the poorly elucidated nature of PDA, and despite the fact that demand avoidant behaviour is something that can come and go across children’s lives. PDA changes how families think of anxiety, and not in a good way. They think their only option is to accommodate the anxious avoidance and before you know it, it’s got worse. We know how to help anxious children - this doesn’t change because that anxiety has been given the label of PDA. That’s why I think clinicians need to start talking honestly about narratives that can harm. We need to stop looking the other way when advocates talk about neurobiological commands, or tell families that their children have nervous system disabilities. We need to read what they are reading, watch what they are watching - and speak up about neuro-deterministic narratives that are based on nothing and can alter the trajectories of people’s lives. One final, horrifying subtext to Miliband’s speech announcing a trade ban on the settlements should not go unremarked. In desperately trying to keep alive the West’s two-state solution, Burnham’s government is not just ignoring reality: the fact that there is no partner in Israel – electable or otherwise – to implement such a vision. Burnham and Miliband are also insisting, in the entirely improbable event that a Palestinian state can be extracted from Israel, that the Palestinian people should be required to make peace with an unrepentant, heavily militarised, racist state that has recently committed genocide in Gaza. None of the 11 states that joined the UK this week in sanctioning the settlements can apparently imagine Palestinians ever living in anything more than a small, demilitarised, economically dependent pseudo-state, with no real choice over its leaders, or control over its borders and economy. Similarly, there is no suggestion that a neighbouring, territorially reduced Israeli state would need to be reformed ideologically, or made to engage in any kind of truth and reconciliation process, or face sanctions until it sees the error of its genocidal ways. The Jewish supremacist ideology of Zionism, the dehumanising logic of the Israeli settler-colonial mindset, and the religious fundamentalism of large swaths of the Israeli population - all critical to the rationalisation of the Gaza genocide - would remain unchallenged. On this view, it seems, long-established impulses towards apartheid and genocide would simply evaporate, even as the West continues to indulge Israel’s every whim. This isn’t a recipe for peace, even of the flawed kind. It is a demand that the Palestinian lamb continue to dwell with the Israeli wolf. The truth that Burnham’s government refuses to face, as with every government before it, is that peace won’t magically arrive with Palestinian statehood. A Palestinian state – or, more likely, a single state embracing Palestinians and Israeli Jews – can emerge only when Israel has been dismantled as a heavily armed settler-colonial state, as happened with apartheid South Africa. Israeli society will need to be re-educated through a painful truth and reconciliation process, as also happened in apartheid South Africa. Israelis will have to be made to confront the facts about a century of atrocities committed against the native Palestinian people. Britain is not contemplating doing any of this – because the threads from unravelling the crimes of Israel and the Zionist movement would lead straight to the door of 10 Downing Street and other western powers.
