Wild: A female Australian news anchor just called the 49ers the 69ers on live television. "We’re watching some footy game in America, the Rams and the San Francisco 69ers play some rubbish game." 😭😭😭

This is an old clip from Australia's Today show in October 2022, not recent. It has recirculated online for years. outkick.com/culture/tv-anc… dailymail.co.uk/tvshowbiz/arti… x.com/Outkick/status…

A little Medicare Advantage history lesson… Medicare Advantage did not begin as Medicare Advantage. It began as Medicare+Choice, created under the Balanced Budget Act of 1997, which authorized private HMOs, PPOs, and other coordinated care plans to contract with Medicare and enroll beneficiaries in place of traditional fee for service coverage. The design rationale at the time was managed care efficiency. HMOs were already displacing fee for service in the commercial market, and lawmakers wanted Medicare to capture the same purported savings. Medicare+Choice underperformed almost immediately. Plan withdrawals were frequent through the late 1990s and early 2000s as insurers found payment rates too low relative to their costs, and enrollment stayed well under 20 percent of the Medicare population for most of that period. Congress rebuilt the program with the Medicare Modernization Act of 2003, renaming it Medicare Advantage and restructuring payment around competitive bidding, risk adjustment tied to diagnosis codes, and more generous benchmark payments designed to attract insurer participation. The 2003 law also introduced Medicare Part D alongside these changes, but the MA payment redesign is what set the trajectory the program is still on today. Benchmarks were explicitly set above fee for service spending in many counties to induce plans to enter the market, on the theory that competition would eventually bring costs down. It did not work that way. By 2009, MedPAC found Medicare Advantage payments already running roughly 14 percent above what the same beneficiaries would have cost in traditional Medicare, driven substantially by diagnosis coding that made enrollees look sicker on paper than they were in practice. That payment gap and its primary cause, identified within six years of the 2003 redesign, is functionally the same gap and cause CBO and MedPAC are still measuring today. The structural problem was present at the onset, not something the program drifted into later. At the end of the day, Congress built a payment structure with the overpayment mechanism embedded in its design, and insurers happily took advantage. Their advantage has led to substantial market consolidation for physicians, insurers, and hospitals in response to market forces. Congressional inaction has contributed significantly to the exponential rise in the cost of healthcare today.

An apt day to remember that Dolly Parton once recorded a cover of Stairway to Heaven - and, in defiance of every moral law in the universe, she absolutely owned it.

I am fortunate to have the chance to visit with so many leaders that are nurses by background and now serve in nurse leadership positions or increasingly as CEOs of hospitals and health systems. Incredibly awesome and inspiring. I am reminded of this every day as I interview great leaders and as I joined the Accenture Nurse Leadership Summit yesterday. Amazing work there by Ajay Mody and Asher Perzigian. Remarkable leaders and professionals. Thank you.

The Odyssey-THE SEQUEL-So this gets really weird! Although Chris Nolan didn't let us see it, Odysseus secretly slept with Circe. I guess it was hard to resist the witch who had turned your men into pigs! And after he ships off to Ithaca, Circe has a son,Telegonus, who goes searching for his dad in Ithaca, accidentally kills him and gets granted immortality by Circe, and then marries Penelope! That's right, marries his dead father's widow. So maybe it was no accident! But that Homer, what a wild, wicked and weird imagination.

Just watched the incredible Odyssey, thoroughly enjoyed but noticed one gaping plot hole. Don’t know if it’s Homer’s hole or Nolan’s. But this: the famished crew slaughters the sun god’s cattle knowing it means certain death, but in a later scene Calypso scoops up a net full of fish with no effort at all. And what about a little hunting for crabs, clams, etc as opposed to dying at sea?

A video that is currently going viral online: the groom’s friends created an incredible clip for a couple from Kazakhstan using AI. It shows the lovers as if they had met in different eras and, across centuries, kept finding each other again and again. The bride’s reaction is priceless.

Is PGA using AI or do they need to: Rory McIlroy, the 2023 Scottish Open champion, posted a 4-under 66 Friday to remain in a tie for the lead and sits at 8 under alongside Tom Kim (66) and England's Jordan Smith, who carded the low score of the tournament so far with a 63. Matt Fitzpatrick (65) and Min Woo Lee (66) sit one back at 8 under.
