What if the outcome of a resuscitation depended less on the algorithm and more on who happened to walk into the room that day? Dr. Charles Roehr describes neonatal resuscitation as a kind of lottery. Every mother baby dyad that comes through the door gets whoever is on shift, and every clinician brings a different read on acuity, a different comfort level, a different instinct for when to deviate from protocol. Guidelines can standardize a lot. They cannot standardize a person. The algorithm ends where human judgment begins. This is why simulation, clear roles, and a team culture where anyone can speak up matter just as much as your equipment. The next resuscitation you run will not look exactly like the last one, and Dr. Roehr argues that is not a flaw in the system. It is the reality we are training for. 🎧 Listen here: podcasts.apple.com/us/podcas… #nicu #neonatal #babies #medicine #newborn #innovation #podcast

New paper from 🇨🇦neo network. Pre-viable PPROM that then make it to >21 wk for resuscitation, increased odds of mortality and morbidities compared to those that PPROM after viability. Not that surprising, but many details in the paper worth exploring. clinicalkey.com/#!/content/j…

I am at about three different stages on this graph at the same time. Several projects all at different stages. No matter the lows, just keep showing up. See you tomorrow.

Follow-up of children born very preterm: the need for evidence-based frameworks to drive best practice and standardised care. This viewpoint discusses the national guidelines for follow-up of growth, health and development of children born very preterm (<32 weeks’ gestation) developed by the Australian Centre of Research Excellence in Newborn Medicine in 2022. In a new paper, I argue that shared decision-making for extremely premature babies rests on a flawed premise: that the "facts" about survival and disability are stable and clear. They aren't — and we need a fundamentally different model. authors.elsevier.com/a/1mxTg…

Incidence of herpes simplex infection among febrile infants under 3 months of age: evidence from two multisite cohorts. In 3733 febrile infants ≤90 days presenting to UK EDs, HSV infection was rare: 0.2% (95% CI 0.0 to 0.8) in those <29 days and none in older infants. This supports current consensus that well-appearing infants aged 29–90 days do not require routine empiric HSV testing or treatment unless specific risk factors are present. However, many neonates with HSV disease, including disseminated infection, present afebrile with non-specific signs of sepsis; therefore, the 0.2% estimate likely underestimates the true incidence. UK Paediatric Antimicrobial Stewardship guidelines recommend aciclovir for neonates with suspected sepsis and any of the following: ALT/AST>2× the upper limit of normal, coagulopathy, vesicular rash, seizures, CSF pleocytosis, suspected meningitis or encephalitis, recent maternal HSV infection or postnatal HSV contact. Aciclovir is also advised for neonates aged 3–14 days with sepsis who have no clear cause, are not improving, or where there is unexplained maternal febrile illness.
