Five things I never let my patients do: Kegels with pelvic floor pain. Shockwave for ED from a radial device instead of a focused one. Ignoring ED in your 40s. Giving up on a vasectomy reversal at 10 years. Skipping the semen analysis after a vasectomy.

The data that @JuanJAndino provided is surprising. I’ve never seen a priapism from Viagra or cialis alone or even together at high doses.
I’m still not sure those patients were just taking those meds alone, I think there is confounding factors we don’t know about, and I’m not very worried about my patients getting priapisms using them. The key of course is counseling.

Its same as of CHECK CYSTO post TURBT
If you have had your prostate removed, somebody decided how often you get a PSA test. Mostly that decision came off a schedule written for everyone.
A Karolinska-led team modelled 11,552 men after radical prostatectomy against what actually happened to them. Median follow-up in the men who never recurred was 90 months. Three quarters had something adverse on the specimen: pT3-4, positive nodes, positive margins, or ISUP grade above 2.
At five years, 78% were free of biochemical recurrence. On the EAU definition of high-risk recurrence, 91%. On the EMBARK definition, 98%.
What the model does with that: men with adverse features want the tests packed into the early years, quarterly for two years and twice yearly out to six, then eased off. Men with favourable pathology did fine at every nine months the whole way through.
Two hazards are moving in opposite directions here. Recurrence risk is front-loaded. The risk of dying of something else climbs with age. Depending on the man's disease, age and comorbidity, the year those two crossed ranged from four to past twenty.
Retrospective and model-derived, so nobody should rewrite a follow-up card on it yet.
