ALT “I work with frail older adults and their families to achieve a safe, functional and sustainable life outside of the hospital environment. I am often making very difficult decisions about people’s living environments and their care needs. I also teach, train & lead a group of therapy staff in a speciality I feel passionate about along side an amazing multidisciplinary team. My aspiration is to inspire others to forge a career working with older adults with dementia and embrace this speciality.”

ALT “I work with a range of patients across multiple specialities, linking in with the multidisciplinary teams in order to achieve safe and effective discharges. Working closely with the families and my community colleagues is vital to ensure that the patient’s needs are being met once they have left the hospital, so that they are safe and supported to continue to lead their lives as independently as possible.”

ALT “I work with adults who have come into hospital to have elective or emergency procedures/operations across a variety of surgical areas. I am looking at how to safely discharge these patients with new and potentially long-term needs and disability. We work extremely closely with our physiotherapy colleagues and the wider multidisciplinary team. My aim is to enthuse individuals in the rehab element of our therapy, especially for amputee patients.”

ALT “I work with a range of patients across respiratory and acute medical wards. I strive to carry out a person-centred approach, identifying individual needs and requirements. I then provide guidance and direction to enable people to continue leading their lives as independently as possible. I enjoy the fast pace of the acute setting and aim to make a positive difference to patient care, including achieving safe and effective discharges.”
