I study administrative burdens in health insurance coverage, strategies to reduce these barriers, and the politics of health reform also @adrianna.bsky.social

When we talk about health insurance premiums, we typically frame them as financial burdens, because they often are! But—absent auto-pay—paying premiums is also a hassle, and I had this nagging question: what do tiny premiums do to enrollment? We try to answer that here. 🧵

The Trump admin announced they canceled ACA coverage for 760,000 people. Insurers were asked to reach out to these people and they didn't respond w/in 30 days. Some were likely fraudulently enrolled, some were likely legitimately enrolled. We don't yet know the collateral damage.

Adventures in "arbitrary and capricious" land!
“What if Trump said, ‘I want to do these [$500 ACA refunds], but only in states that voted for me in the last election’? What is stopping him from doing that, but not stopping him from doing this?” statnews.com/2026/09/10/trum…

No state has yet been successful in partially expanding Medicaid — that is, pulling down enhanced match for enrollees up to 100% FPL, while keeping 100-138% FPL folks in the Marketplaces — but Utah came close in 2019 under the prior Trump administration politico.com/story/2019/03/2…

Sad to share that Peter Barton Hutt, the longtime food and drug lawyer who charmed his Harvard students (and our readers) with scorpion lollipops and the code of Hammurabi, passed away this morning at the age of 91, according to a family member. wsj.com/health/calories-food…

Georgia is facing a Medicaid funding shortfall because the state has to adapt its work requirement policies to new federal standards, which are more lenient than what the state had implemented and budgeted for in their partial expansion (receiving only standard FMAP).

Back in 2013, GOP officials in AR made the case for budget neutrality of its "private option" 1115 waiver by arguing the state would otherwise need to stand up a costly new managed care regime to expand Medicaid. Now it's the first state to face waiver non-renewal under OBBBA.

You can argue that that's a lousy counterfactual (some did back then). But it'd be wild to force the state to quickly push their entire expansion population into FFS amidst all of the other policy changes happening in tandem (eg, work requirements in 2027, cost-sharing in 2028).

Now that he's secured the nomination, I do wonder if we're going to see AES's discussion of Medicare for All take a more pronounced incrementalist posture, leaning into affordability and populism — rein in prior auth, reduce deductibles, break up big corps

I’m stuck on the presumption that one-third of parents believe their children can afford routine therapy. In this health care system?
It's so weird that the Roman Empire fell because of something Katie Miller was already mad about. (Also, why the parents would be a reliable source here is a question that should be raised)
