I love getting subtly roasted by my attendings.
“I think the real reason you get along so well with geriatric patients is because you have all the same hobbies as them.”

I’m a primary care doctor. I care deeply for my patients, but I need to say:
No, I can’t do anything if your insurance company denies your GLP-1.
If a portal message requires more than 10 words or more than a minute of my time, it should be an appointment.
Plz just exercise.
Above all else, I care. Like, really truly care. I have dreams about my patients on a regular basis & am constantly thinking about what I can do to improve their health & overall quality of life. I prepare jokes for specific patients who I know need to laugh & make birthday cards

This is Tommy. Tommy had a question about my allegiances. Listen to the end to hear about his.

He stopped the shooter, but he didn't stop the shooting. That's the difference. Three kids and their teacher were already dead, and an unarmed teacher had tackled him first. YOU spent decades blocking laws that keep guns away from kids. Now you're taking a victory lap? Fuck that.
*patient spends 40 min new patient appt talking down to/over me & explaining to me why statins are evil despite hx of advanced coronary artery disease*
Patient, as I’m leaving: “So what are you? A nurse or a PA or something?”
Me: *sighs in 3 degrees & 13+ years of MD education*

As an outpatient physician, this is the most misunderstood conflict between patients and doctors.
From the patient perspective, it’s inconsiderate, especially when many offices will reschedule if you are more than 10 minutes late. Traffic, child care, finding a ride, time off work. So many factors go into just getting to a doctor’s appointment on time.
From the physician perspective, there is a giant machine behind the doctor dictating how we run our clinics. Administration, reimbursement, regulatory hurdles like prior authorizations, balancing wait times with patient demand. If you asked most physicians how many patients they would like to see in a vacuum, most would choose a number that is far fewer than their current volume, but the healthcare system has other ideas.
I wish we had an empathy contraption. Two people wear it and you instantly understand the other person’s experience. In lieu of that, these types of complaints and disagreements can be remedied with better communication. Give patients updates on the status of the clinic. Vast majority of people are reasonable, just communicate. Have a system for it.
If lawyers bill by the minute, I should be able to bill for answering inbox messages that should be an entire appointment.
*meets a famous tv/music celebrity*
Me: “Who are they again?”
vs.
*seeing my friends get published in medical journals, meeting my hero physicians I’ve looked up to for years, seeing colleagues absolutely ROCKING caring for patients*
Me: *fan-girling SO hard*
I’ve recently started making “welcome to girlhood” packages for my m➡️f trans patients and honestly, wish I had done this sooner.
Bath bombs, face wash, moisturizer, retinol+niacinamide+hyaluronic acid serums, sunscreen, face masks, hair products etc. +instructions 🥰💅🏼
