Exercise only really works for performance and injury prevention. Don’t sell it for what it’s not

80 year old referred for gluteal tendon pain confirmed via U/s. Clinical ax showing 100 deg hip F, 5 deg IR & 20 deg ER. Positive FABER lat & posterior hip pain replicating symptoms No wonder we see studies showing those with gluteal tendinopathy have a risk of developing hip OA

Hint: it’s actually hip joint pain being misdiagnosed as gluteal tendinopathy

I see this as a platform for exchanging ideas and concepts. Personally, I’m much more interested in the content itself than in the person delivering it. Maybe, though, there’s something about it that makes it easier for people to disengage (take the blue pill)…

avoiding the uncomfortable realization that much of what we do is, in a sense, just distracting the patient while nature takes its course

Do any physios on here go to see a Physio when they are in pain? Do any physios work on changing their biomechanics when they get injured/sore? What does this tell us about what’s important for pain?

One of the common beliefs is that by keeping our joints strong and mobile through regular exercise, we can prevent arthritis. What I don't quite understand, though, is why we still develop arthritis in our hands despite constantly using and exercising them

Frozen shoulder pain is caused by inflammation in the shoulder joint, generating nociception. Early-stage frozen shoulder can mimic rotator cuff related pain (RCRP), suggesting that RCRP may be over-diagnosed, when the pain is actually due to joint inflammation instead

Is it possible that individualization of exercise for pain treatment is part of the grand theatrical placebo that is Physio?

So how do we treat the inflammatory process?
Replying to @Retlouping
Yet many of the exercise based interventions approach it as a mechanical issue.

Had some dental work done. Didn’t sleep well in the days leading up, stressed & anxious about the procedure & pain. On the day, dentist used local anaesthetic, no pain during the whole procedure. Strange isn’t it, once there was no nociception all the other things were irrelevant

This paper suggests that a strong therapeutic alliance is a predictor of reduced back-related disability regardless of who is delivering the treatment. bmjopen.bmj.com/content/bmjo… But how do you achieve a strong therapeutic alliance? A thread 🧵
