First RCT results of Etentamig (BCMA bispecifc antibody) presented at Plenary Session #IMS26 @Myeloma_Society @PlasmaCellPete @myelomaMD 60 mg fixed dose every 4 weeks makes this the easiest bispecific to administer. Impressive PFS benefit in relapsed refractory myeloma with at least 2 prior lines of therapy. See thread for more details.

One of the most amazing things I’ve ever seen: a standing ovation for the full Daraxonrasib results I feel inspired and energised, to put it mildly — we have a targeted therapy for pancreatic cancer now, and nothing is undruggable anymore

How to manufacture a non-causal association: 1. Have authoritative person say “[thing] is good/bad” 2. Wait for smart, conscientious people to do more/less of [thing] in response 3. Boom! [thing] now correlates with every good outcome. Pick one that sounds plausible
Fun fact: Sugar consumption only became associated with bad outcomes after healthy people started taking the government's advice to avoid sugar. The harm signal is a result of selection based on differential uptake of the belief that sugar is bad!

Cudowny lek, trikafta (u nas kaftrio) de facto wydłużyła życie ludzi z mukowiscydoza do poziomu zbliżonego do populacji ogólnej.
Trikafta is one of the most inspiring medical advances in decades. Approved in 2019, the drug at first makes cystic fibrosis patients vomit up mucus for a whole day. But while they lose that day, they gain a lifetime–a healthy, *normal length* lifetime. It's a miracle drug.

I agree with the push for better precision medicine. I think such a future will be invaluable. But the way NNT is being used in this post is a gross misrepresentation. NNTs for the top 10 drugs come almost entirely from short trials, usually 2 to 5 years, while people live with these diseases for decades. An NNT of 25 over a 3-year trial does not mean only 1 in 25 ever benefits. It means 1 in 25 benefited within that narrow time window. Extend the timeline and NNT improves, sometimes exponentially, because absolute risk reduction compounds with age and exposure time. So taking short-term NNTs and using them to imply that most drugs “don’t work for the majority of people” is deeply misleading. It's unfortunate because when someone with a large platform posts statistics this way, it puts them in a tough bind. Either: (1) they don’t actually understand how NNTs and time-bounded risk reductions work but speak on them with confidence, or (2) they do understand and choose to frame them in a way that misleads the audience about what the data really shows. Neither option is a good look.

Vive l’empirisme! nejm.org/doi/full/10.1056/NE…

This is a major story from #ASCO25. Randomized phase 3 trial of time of day of immunotherapy infusion. Randomized to infusion before or after 3pm. Early infusion far superior: PFS 11.3 vs 5.7 HR 0.42, OS HR 0.45! Impactful, pragmatic, not costly. This should be a bigger story.
