Darolutamide with ADT and locoregional radiation therapy (RT) in high-risk prostate cancer (DASL) showed Darolutamide reduced the risk of relapse and time to CRPC. Tolerably was good. It’s imporant to identity which patients benefit, as mortality is low, and whether latest staging with PSMA-PET changes things #ESMO26 @Silke_Gillessen @ChrisSweens1

🚨 VOLGA: chemo-free durvalumab + enfortumab vedotin beat surgery alone in muscle-invasive #bladdercancer 🚨 @NEJM today 🧵1/2 👥 n=695, cisplatin-ineligible or declined MIBC 📊 Phase 3 RCT, 1:1:1 💊 Perioperative durva + EV vs durva + tremelimumab + EV vs radical cystectomy alone ⏱️ Estimates at 24 mo ✅ Durva + EV EFS 63.5% vs 45.1%, HR 0.51 (0.38-0.67), p<0.001 ✅ OS 78.4% vs 69.4%, HR 0.58 (0.40-0.83), p=0.003 ✅ Durva + treme + EV EFS HR 0.61 (0.47-0.80), but grade ≥3 AEs 79.5% vs 71.1% ⚠️ The 2 durvalumab arms were not compared head-to-head, so the CTLA-4 add is not clearly worth the added toxicity 🎯 Another cisplatin-free ADC + IO backbone for cisplatin-ineligible MIBC

Love to see this. We need more discussion of men who are unlikely to benefit from salvage XRT. Such discussion is basically absent from AUA guidelines.
Following RALP for pT2 R0, Gleason 4+4, iPSA of 25, PSMA N0M0, primary tumor PSMA avid: first PSA =4, 3mo later it is 10ng/ml. New PSMA PET is negative. Man is 73yo and fit. What should we do?

In a 10-yr follow-up of a randomized trial, Retzius-sparing RARP (better early continence) showed worse long-term cancer control than the anterior approach — 2.64x higher progression risk after adjustment. Post-hoc, hypothesis-generating. free link:authors.elsevier.com/a/1njm6…

After receiving some questions about our recent @CCR_AACR paper that was mentioned a few times at last week’s @US_FDA workshop on NMIBC trials, it feels important to highlight our data that demonstrates exactly why randomized trials are critical to determine the true clinical benefit of any NMIBC treatments and why we urgently need to move beyond single arm trials for NMIBC drug approval 1/n @ElaineChangMD @chana_weinstock @BladderCancerUS @UrologyMSK @bbmdmsk @bca_md @h_alahmadie aacrjournals.org/clincancerr…

🚨CAN-2409 + RT: not just delaying PSA failure, reducing residual tumor in the gland.🚨 @TheLancetOncol 👉Phase 3, double-blind RCT 🧑‍🤝‍🧑745 pts w/ int/high-risk localized #prostatecancer ⚖️EBRT + intraprostatic CAN-2409/valacyclovir vs placebo. 💉CAN-2409 is a replication-defective adenoviral HSV-tk gene therapy injected into the prostate. 💊Valacyclovir acts as the prodrug to drive local tumor-cell kill + immune priming 📉 DFS improved: HR 0.70; median NR vs 86.1 mo. 🔬 Key nuance: DFS included local failure by centrally reviewed positive prostate biopsy, protocol biopsy ~2 yrs post-EBRT. 🎯 At 22–26 mo: positive biopsies 20% vs 36%; pCR 80% vs 63%. 🛡️ No major toxicity penalty: grade ≥3 TEAEs 8% vs 7%; no treatment-related deaths. 🔑CAN-2409 may not just suppress biochemical recurrence, it may deepen local tumor eradication after RT. @PCFnews @PCF_Science @urotoday @UrologyTimes 🔗shorturl.at/o3ZUq

Discussion around PROTEUS highlights lack of consensus in accepting MFS as a surrogate endpoint for OS for perioperative systemic therapy trials. hopefully this will drive trials to follow long enough to get OS.
4/n #ASCO26 The ICECaP group (@ChrisSweens1) showed using semi-newer trials than the original publication, a slightly weaker treatment effect correlation of MFS and OS, with a surrogate threshold effect (STE) of 0.81 (more on this shortly!). Notably almost none of these trials are surgery trials. The only surgery trial of RP+/- systemic therapy included was CALGB90203. Surrogacy is treatment and disease state specific. Ignoring this, MFS was based on conventional imaging. @AmarUKishan @I_Migowski @TylerSbrt

*maybe* FDA will approve based on MFS, but significant doubt will remain among treating docs (myself included) without OS data -- even if we heed @DrSpratticus's main point here and use conventional imaging to define MFS.

JUST OUT at #ASCO26 with a concomitant Publication in the New England Jouranal of Medicine: TALAPRO 3 :PARP and Androgen-Signaling Inhibition plus ADT in Metastatic Prostate Cancer by our dear friend and OncoAlert founding faculty @neerajaiims 🚨 buff.ly/gk9AuvF In this phase 3 trial of patients with metastatic androgen pathway modulation–sensitive #prostateCancer and homologous recombination repair gene alterations, adding talazoparib to enzalutamide significantly improved imaging-based progression-free survival versus enzalutamide alone (3-year rates: 77% vs. 56%; HR 0.48, P<0.001). An interim overall survival analysis favored talazoparib but was not definitive. Toxicity was higher with talazoparib, particularly grade ≥3 anemia, with two treatment-related deaths reported. @neerajaiims @AzadOncology @quimmateo @nealshore @cvulsteke @montypal @crisbergerot @DrDanielHeng @apolo_andrea @DrChoueiri @PGrivasMDPhD @TiansterZhang @HHammersMD @ravikanesvaran @neerajaiims @amerseburger @sonpavde @drenriquegrande @scserendipity1 @Silke_Gillessen @EfstathiouEleni @tompowles1 @BraunMDPhD @nataliagandur @cdanicas @brian_rini @AOmlin @yekeduz_emre @DrYukselUrun @nataliagandur @realbowtiedoc @AmandaNizamMD @scocmem
