Proposed change to CMS TAVR NCD: Surgeons no longer required to be present for the procedure. Other big changes. Is it good for patient care?

Anybody else seeing a ton of pericarditis lately? I’ve seen 4-5 cases in the last month. Wondering if it’s part of a larger trend. #cardiotwitter

NIH has issued ~65% fewer (796 vs. 2300) new grants so far this year compared to the same point in prior years.
Combined with the delayed release of already-appropriated funding and we are witnessing unbelievable real-time damage to the U.S. scientific enterprise.

Chronic stent recoil occurred in 1 of 5 lesions treated with second- or newer-generation #DES requiring repeat #revasc, with severe calcium and nonuniform stent expansion identified as risk factors, and was associated with TLR. jacc.org/doi/10.1016/j.jcin.… #JACCINT #OCT

Welcome thoughtful National Coverage Analysis on #TTVR published yesterday. Access to healthcare is critical and @CMSGov decision is a testament. Great job! Treatment for TR enters a new era! @SCAI @ACCinTouch cms.gov/medicare-coverage-da…

Congrats to Howard Herrmann, @Drroxmehran @didier_tchetche and #SMART trial investigators for outstanding 2y data and outcomes #CRT2025, with excellent clinical and echo follow up! While no differences in hard endpoints (no surprise), what struck me the most is the adjudicated prosthetic valve thrombosis differences (clinical, subclinical) between #Evolut and #Sapien3 #TAVR, despite no differences (yet) in reintervention. But it reaffirms to us that BVD no matter how it’s defined is not a good thing for prosthetic valves, whether it’s TAVR or in the latest @JACCJournals paper by @StevenYakubov et al, SAVR. @MDT_StructHeart

Cutting NIH to save budget is like cutting the front lobe of brain to lose weight. It’s <1% of body weight yet it governs reasoning and problem solving functions of the brain, basically determining how smart you are. Dumb move to send IT kids to do your brain surgery, America.

Back to ECMO here in @GaHeart_NGHS and love it. Here’s why a retro perfusion cannula (that I started in @cusm_muhc) is better than an antegrade (see below)

"In interventional cardiology we will use the metric system to measure things."
"Like catheter diameters Sir?"
"No - those we will measure using French sizes."
"What about wires Sir?"
"We will measure those in inches"
"Why Sir?"
"No one knows"
