Great tutorial. As an old-timer who still performs open surgery, I’m surprised by the clipless approach. I prefer to clip the major lymphatic vessels. Who else? #urology

Enblock resection of bladder tumor with holmium
youtu.be/Gr9WytM-orw?si=3saI…
En bloc resection of bladder tumours can be particularly useful for lesions at the bladder dome, where a TUR loop may make controlled resection with an adequate deep muscle biopsy challenging.
Holmium or thulium fibre laser allows precise depth control—reported down to 0.1 mm. Sharing a video of the en bloc resection of an exactly 2 cm tumour located at the bladder dome.

Delighted to share our latest publication!
Nerve-sparing prostatectomy is well established, yet ED after RP remains common.
The idea of arterial-sparing RP is not new, but traditionally relies on intraoperative identification of accessory pudendal arteries.
Could preoperative mpMRI guide arterial preservation?
Our study suggests potential.

Lymphocele cited as major reason in guidelines to avoid LND in prostate cancer. Here we show most almost all lymphoceles (42/43) come from Cloquet's node, which should not be removed during LND. auajournals.org/doi/10.1097/…

Traumatic catheterization is a reason to postpone intravesical BCG. BCG contains live, attenuated mycobacteria. If catheterization leaves the bladder lining bleeding, they can spread beyond the bladder and cause systemic infection.
Visible hematuria and symptomatic UTI are also contraindications. Microscopic hematuria or an asymptomatic positive culture alone aren't, according to EAU.

Histologic diversity in #BladderCancer is stage-dependent: rare in pTa (<2%) and pT1 (<5%), but common in MIBC/mUC (~35% variants/divergent). Squamous differentiation dominates (~90% of divergent cases), with basal-like biology and distinct therapeutic targets.

Pelvic node dissection gives the most accurate staging available, but neither randomized trial moved PSA relapse. The review from @AshTewariMD's department argues those nodes are where T cells get primed.
