When the ureteral orifices sit close to the bladder neck in a large, broad-based, congested prostate, visibility and traction can become challenging.
A useful HoLEP strategy: split the adenoma in two. Better visualization, less traction, and easier identification of the ureteral orifices.
This case: 120 cc prostate, urinary retention, and metastatic prostate cancer.

Enucleation is only if tissue is enucleated not resected ! This a good surgical video to demonstrate this . And if one leaves such tissue patinet will come back .

A patient with a history of Rezūm and TURP is still voiding terribly, despite alfuzosin + finasteride. And now, as a third step, he’s being offered prostatic artery embolization (PAE). 😲
What about #HoLEP? Why not?
We should offer patients what they need—not just what we have or what we know how to do. 😀
