How do you manage immunosuppression in dialysis-dependent glomerulonephritis? 🛑STOP: When risks outweigh benefits (e.g., infections, malignancy, fatal drug interactions). Primum non nocere. ⤵️TAPER: When renal recovery is unlikely (e.g., extensive IF/TA or advanced irreversible Anti-GBM injury), or to prevent rebound complications (e.g., prolonged corticosteroid therapy). ✅CONTINUE: If renal recovery remains possible, extrarenal disease is active, or to reduce post-transplant recurrence risk in selected active diseases.
