This happened because of
1. a total failure of, or a total lack of internal processes and systems
2. Zero efforts at auditing or even basic checks to see if anything appeared amiss
3. an entire galaxy of incompetence by all involved
Absolutely no way any of this catastrophic failure can be explained away by mistakes or other minor items.
Global TLS Intermediate CA incident: We’ve updated our incident page with how it happened, who is affected (one US customer ecosystem), what’s not affected, and what we’re putting in place. We will publish the root cause once our investigation is complete.

Readers added context they thought people might want to know
eMudhra minimizes the scope by claiming only "one US customer ecosystem" is affected. In reality, 8 of its CA certificates had catastrophically weak private keys (one being simply the number 4), allowing anyone to forge trusted HTTPS certificates for any website on the internet.blog.google/security/prote…

Bandwagon companies. Those that magically appear when new shiny things are launched with grants for all. Usually claiming to be experts in whatever it is.
Sling it in, grab the grants and then disappear, leaving others to sort out the mess. – at Poole, England

Today, not so much.

Proprietary, expensive, closed system bullshit which will be obsolete as soon as the manufacturer has something even more expensive to shovel out.

It's so you are compelled to take it to the main stealers, who will charge you vast sums just to tell you it now needs even more spending on it.
Their entire business model is built on expensive servicing and repairs. That warning light is how they suck people in.

BBC might as well be renamed the GBC, Government Broadcasting Corporation, dispose of the licence fee, and be funded from general taxation.

Doesn't really matter.
Punctures are rare events, and in plenty of those no repair is viable due to location of the damage, or as in that photo, the tyre is already EOL.
In any case, repair involves removing the tyre from the rim so it can be inspected internally. Those shove it and plug it efforts are absolute last resort for a temporary repair to allow travel to a nearby repair emporium at low speeds.

The deal here is that the restaurant has created a loss making item for the 'child' menu, as a smaller portion doesn't halve the cost of providing it. Probably done originally as a promotional item with the expectation that every child would be with at least one adult and in many cases two.
Then they discovered that some adults were buying the 'child' option because they wanted less food, and so the whole thing became a loss making affair.
Their solution was this shambles based on height.
The real solution is to dispose of the 'child' menu option and have the same prices for all. They can ditch the '3% service charge' at the same time and just add that to all of the prices, and ditch the 'everyone must buy food' malarkey as well.
Then people can buy what food they want, possibly share one or more dishes if they desire, no one gets bent out of shape and the final bill will be the prices quoted on the menu.

The only place you can go the same distance North, East, South and arrive at the same place you started is the South Pole.

Total BS question. Implies being at the pole, where there are no woods, there are no cabins to rent there, hunting is unlikely to be permitted and only morons would chase a bear even if one existed there, which of course it didn't.
Those that posed the question should be fired.

1 - that can never be made impossible
2 - by the time someone has noticed the problem and obtained the gun, it's already too late.

Or - don't bother paying any breakdown cover company at all. Just arrange and pay your own recovery on the rare occasions that it is needed.
