📰In our latest Health InterACTion, CHN celebrates 10 years of our ACT/SNSW HealthPathways program. In this period, over 670 HealthPathways have being developed, which have supported approximately 6,000 primary health care professionals with condition-based assessment, management and referral information at the point of care.
This milestone is a testament to the dedication, collaboration and innovation of the ACT/SNSW HealthPathways team. There’s so much to be proud of!
