FFR is a surrogate for ischemia. Ischemia is a marker of adverse outcomes, not a cause. Revascularization of individuals with ischemia does not improve outcomes on OMT. Why do we still obsess over FFR and its derivatives?

13/15 We thank everyone who were part of this. Please share our work widely, so that the public/patients get what they truly deserve. High time to stop blindly following what pharmaceutical industry has been making doctors do what they do like sheep. Time to make changes.

The reason why NI trials use a combined efficacy and safety NACE outcome is because it is typically biased towards NI (outcomes going in opposite direction cancel each other out!). Journals should know better, yet they publish these substandard studies.

I will start another debate on left atrial appendage occlusion. This one in Hyde Park format at #HRS2024 I have 9 reasons why I am not convinced this procedure benefits patients. This is percutaneous closure. Surgical closure was shown effective (along with AC) in LAAOS3 1/

"...fewer events than anticipated in the surgery group..." is a strawman! Why would one expect SAVR rate to be higher than TAVI in this cohort with 1/3rd sample with bicuspid AS? If they had considered equivalent rates (10% for both TAVI & SAVR), N would have increased 4-fold!

NOTION-2 did not demonstrate the noninferiority of TAVI compared to surgery for the primary endpoint at 1 year. Due to fewer events than anticipated in the surgery group, the trial lost statistical power, resulting in widened confidence intervals that crossed both the noninferiority boundary and the line of identity. This is the methodological situation where a noninferiority trial is termed "Inconclusive" because no result can be significantly excluded. We cannot say TAVI is noninferior, we can't say it is inferior, we can't say it is superior. Check mate. That said, NOTION-2 remains valuable for descriptive purposes and inclusion in meta-analyses. Long-term results with increased event rates will provide additional insights on the comparison of TAVI and SAVR in younger, low-risk patients. academic.oup.com/eurheartj/a…
