Why are practitioners still doing fluroscopic guided shoulder joint hydrodistensions for A capsulitis ? Any ideas @DrJN_SportsMed - four times the cost of US-guided without radiation. What am I missing here?

Private medical insurance companies - a few tips for you..... If you intervene & send patients to another scanning centre against my knowledge - 1. The pt gets a substandard quality scan 2. The pt doesn't get the imaging, series & protocol I specifically requested - this is a waste of everyone's time because it has to be repeated eg - MRI pelvis is NOT the same as MRI hip - wide FOV is not as good as small FOV And completely ignore my referral request for xray too 3. I'm not given the report & I don't have access to the images - extra admin work chase-up +++ & embarrassing when the pt comes for their FU appt @AXA @Vitality_UK , I'm looking at you. Start prioritising your pts care, not embarrassing attempts to manipulate your financial flow pathways, it's terrible care....

Femoral neck stress #s /injuries are high risk injuries and the outcome depends on early diagnosis. Working as FCPs and in community MSK clinics one does come across runners presenting with anterior hip pain. Hereโ€™s a slide with some useful information on it #bonestressinjuries

WARNING! Do you run webinars? @WebinarTVus are stealing @Zoom recordings & uploading them to their platform! Happened to me yesterday - I was emailed by **Sarah Blair** to say theyโ€™ve taken a recording of one of my public webinars, edited it & published on their site. WITHOUT my consent. Google it & thereโ€™s heaps of complaints - same for meetings & conferences. They fail to respond to emails & requests to remove. @Zoom still to respond to me. From now on I will not permit AI assistants & hope that webinar providers will up their game. This is SO not okay. Share widely & be vigilant @AdamMeakins @CorKinetic @ShoulderGeek1 @tomgoom @physiojack @TPMPodcast

I keep trying to reinforce this point - If you want to evaluate acetabular dysplasia, you need a good quality WB plain film for the pelvis, with MRI providing information about 'surrogate' or associated pathologies eg labral tears / hypertrophy, capsular thickening, ligamentum teres changes, chondropathy.
Said it before, do not rely on a hip MRI report (or even an MRI arthrogram report) alone. This report was read as "normal acetabular depth", when there is clear bilateral hip dysplasia. X-rays and MRI's are complimentary tests. #HipArthroscopy #HipPreservation

The 'it doesn't really matter' approach is exactly why we see runners doing endless foam rolling and ITB stretches with poor outcomes. Understanding pathomechanics is fundamental to targeted interventionโ€”that's literally what evidence-based practice is." Knowing it's compression helps us modify training loadsโ€”shorter stride length, avoiding camber/hills, and graduated return to flexion-heavy activities rather than just 'stretching the ITB' (which we know doesn't work anyway).
Does anyone have thoughts on how understanding the precise mechanism and anatomy of ITB pain helps with management? Whether its compression or friction (currently unknown) it really doesn't inform Physio

Understanding Iliotibial Band Pathology: When discussing the iliotibial band, it's important to note that although its anatomical insertion is at Gerdy's tubercle, this isn't typically the site where pathology develops. Instead, problems often arise at the level of the lateral femoral condyle, particularly with repeated knee flexion. Long-distance runners** are a classic example. With ongoing flexion, the vastus lateralis muscle compresses the fat pad situated between the iliotibial band and the lateral femoral condyle. This compression leads to changes in the fat pad, often observed as echogenic and hyperaemic fat on imagingโ€”hallmarks of iliotibial band compression syndrome. Not Just Frictionโ€”It's Compression: Itโ€™s crucial to clarify that this condition is not a friction syndrome, as is commonly believed. Rather, it is a *compression syndrome*, with the underlying mechanism being the direct pressure on the fat pad rather than friction between the band and bone. Understanding the precise location and mechanism of iliotibial band pathology helps us better diagnose and manage athletes and active individuals who present with lateral knee pain.
