This is a paper everyone needs to read .. it may ( with time and validation) change our field significantly in both NMIBC and MIBC management couple 💭 - HUGE benefit of cfRNA is 0️⃣dependence on tissue .. anyone trying to do utdna on Ta/ small focus HG/ CIS knows how valuable this is but utDNA tumor informed=cfRNA - bcg prediction is immature and underpowered but strong signal - ❗️consider false positives in here 11% in noncancer ctrl. Promising results nonetheless and better than anything we have out there .. - no validation / testing post tmt the most difficult cohort for surveillance with urine based testing and that is why we @CleClinicUro are excited to partner with @max_diehn and ReseroBio to continue their validation
Urine cfRNA is emerging as a powerful bladder cancer biomarker: 95% sensitivity at 90% specificity, with potential for MRD detection and BCG-response prediction. Prospective validation comes next. @NatureMedicine @OncoAlert @OpenMedicineHQ @isliquidbiopsy

When "cancer" is a diagnosis but not really a disease A slightly more philosophical piece than my usual—on what we mean by cancer, and whether the word itself can sometimes do more harm than good.... let me know what you think! Many thanks @pam3001 @aeonmag aeon.co/essays/we-need-a-bet…

Your patient has had nephrectomy for high-risk clear cell RCC. The next decision: Surveillance, Pembrolizumab, or Pembrolizumab + belzutifan. Is adjuvant justified when surgery may already have been curative? Our new commentary explores this dilemma. 🧵bit.ly/4jkism6

Important change in the 2026 @AmerUrological NMIBC Guidelines: all high-grade Ta tumors are now classified as high-risk. This reduces a long-standing source of heterogeneity in the intermediate-risk group and should improve risk-appropriate treatment for patients, while creating cleaner, more meaningful populations for clinical trials. Great to see the evidence translate into practice. @IBCG_BladderCA @UrogerliMD @pjhensley11 @AmirHorowitz @mouwlab @spsutkaMD @shilpaonc @AndreaNecchi @LAUrology_NL @karima_oualla @PGrivasMDPhD @marigfern @SpiessPhilippe @UroCancerMD
Time to revisit something I’ve been saying for years: TaHG bladder cancer is not intermediate risk. The clinical outcomes, molecular biology ... all point in the same direction. Yet some frameworks allow < 3cm TaHG tumors to be downgraded into “intermediate risk” alongside low-grade disease. That classification has real consequences for patients: • undertreatment • less rigorous surveillance • in some studies, these patients are even placed on observation as a supposedly appropriate “control arm.” TaHG is high risk. 🧵 @BladderCancerUS @WorldBladderCan @IBCG_BladderCA

Big week for #prostatecancer news @JAMA_current! First @grace_syl for PROFOUND-VET: use of #activesurveillance is now 93% for low-risk disease and 62% for favorable intermediate. Huge news and proof that @VeteransHealth is leading the way in this area. dx.doi.org/10.1001/jama.2026…

🚨 Half of focal therapy for #ProstateCancer in the US is happening where no guideline supports it 🚨 @JAMA_current research letter 👥 1,179,384 pts w/ nonmetastatic PCa, NCDB 2010 to 2023 📊 15,672 (1.3%) got focal therapy ⚠️ 51% was in low, high, or very high risk disease. -No guideline supports routine use in ANY risk group outside trials or registries ⚠️ Low risk held flat (1.8% to 2.2%) and favorable intermediate is rising (2.1% to 2.9%) while marketing and&reimbursement expand ⚠️ Focal therapy more likely w/ age ≥75 (adjusted probability 22.9%), community centers (7.6% vs 2.5% academic), nonprivate insurance ✒️From the accompanying editorial "Volume of this kind shows diffusion, not benefit." 🎯 Key points 1⃣In low risk this converts surveillance candidates into procedure pts 2⃣In high risk it is undertreatment. 3⃣Repeated use is building "an aura of legitimacy the data have not earned." We owe pts trials, not marketing. 🔗 Study: doi.org/10.1001/jama.2026.12… 🔗 Editorial: doi.org/10.1001/jama.2026.16… @QDTrinh @DrSpratticus @AmerUrological @UroOnc @SUO_YUO @urotoday @UrologyTimes @PCF_Science @PCFnews

In this article @Annals_Oncology GLP1 receptor agonists reduce cancer incidence. The focus is on those cancers linked to obesity @AparnaKamatMD . A retrospective study with a large sample size in people looking to lose weight . A detailed case/control type method. 41%⬇️ in cancer with GLP1 injections. It’s not clear if the effects of wt loss or the agonists are having an effect. Validation needed. Great work!!

🚨 PROTEUS: perioperative apalutamide moves into high-risk localized #prostatecancer Just out in @NEJM 🧬 2109 men w/ high-risk localized or locally advanced PCa randomized to ADT + apalutamide vs ADT + placebo around radical prostatectomy. 🎯 Both stated primary endpoints met. ⚠️ But the trial deserves nuance. RAS finally getting drugged is one of the great stories in modern biology, and almost nobody outside oncology understands why it's such a big deal. YOU'LL LEARN SOMETHING AWESOME TODAY. i am going to keep this as understandable (and simple) as i can. OPEN THE THREAD. 🧵
