💡 Caseous necrosis at the aortic valve is far rarer than at the mitral valve, with only isolated case reports.
It looks like an echogenic mass with a lucent center near the AV (toothpaste-like), so it can be easily mistaken for an abscess, tumor (PFE), thrombus, or vegetation.

The pulmonary valve isn’t called the “neglected valve” for nothing.
In this sexagenarian with a BAV, it’s easy to focus all your attention on the aortic valve - and miss the pulmonary valve pathology.
Don’t forget to look beyond the obvious. #echofirst

KDIGO is going regret that definition of AKI for years. In this study dapa prevented AKI by not tripping over the oliguria trigger for AKI.
You don't say? The diuretic helped prevent oliguria. Unthinkable?

Replying to @DHZB_Akademie
@DHZB_Akademie "3D Advanced Echocardiography Workshop" (happening January 22- 24, 2027) makes it clear why this is way "more than just 3D."
The program bridges advanced imaging with intensive clinical realities”specifically targeting ICU management, complex valvular heart disease, mechanical circulatory support (MCS), and heart failure.
Perfect for clinicians who want practical utility, not just pretty 3D pictures.
Yes, it’s a 3D workshop but we are taking it straight to the ICU
At the #echofirst Workshop , we aren't just looking at static valves. We’re tackling the tough cases:
Echo in ICU: Deep dives into ECMO hemodynamics and using echo to guide acute fluid management.
Critical Decision Making: Using acute heart failure echo to trigger therapy (from diuretics to MCS).
Live Knobology: Real-time mastering of GE & Philips systems.
Register & view the full program: backend.dhzb-akademie.de/app…
Just take a look at this GOAT line up 🔝

Recommendation of the day: If you're preparing for advanced peri-op #TEE #echoBoards, follow @alex1708ander.
His cases are excellent for training your 👁️ to recognize a wide range of pathology. I wish I had found his handle before my exam, but I'm sure it'll be a valuable resource for the next generation of #POCUS enthusiasts.

We've heard about atrial septal pouches.
We may have even seen one.
But, did you ever think you'd see a thrombus-in-situ within an atrial pouch?
CASE reports help expand our understanding of uncommon events.

✅ Echo imaging of TAVI endocarditis is difficult
✅ Even with TEE, IE diagnosis can be challenging due to reverberation artifacts from TAVI components (the frame, skirt etc)
✅ An example of TAVI IE ⬇️. At the cusp level, no IE mass seen on TEE. Mass was below cusp level & within the valve frame. Easily shown by SAX view at subvalvular level (3rd clip)

PR velocity can be used for assessing LV as well as RV pressures. The nice PR signal shows high early diast. velocity indicating PHT. Initial PR slope drops quickly indicating rapid rise of RV diastolic pressure, followed by late diastolic rise, indicating PAEDP > RVEDP from increased LVEDP. Would have been nice to have an ECG. Please see simultaneous PR CW Doppler and LV/RV pressure tracings(in A Fib), below.
EF ~20%, severe MR/TR. PR CW Doppler demonstrates late-diastolic notch with re-acceleration. What do you think is the main mechanism—backward pulmonary venous pressure transmission or something else? @argulian @JaeKOh2 @CASivaram1 @DrRajeshG1 @echo_batman
