Pentagon probe found that the Feb. 28 US missile strike on an Iranian elementary school in Minab — killing 123 children — resulted from a cascade of failures.
Outdated intelligence that still classified the school as an IRGC facility, compressed timelines as the administration demanded 1,000+ strikes in 24 hours, overreliance on Palantir's Maven AI system (which personnel wrongly expected to flag stale data), and Hegseth's 90% cuts to civilian-harm-mitigation teams.
The site had visibly operated as a school for nearly a decade, was on Google Maps, and had its own website.
UN report this week said the failures "went beyond negligence" and may constitute war crimes.
The Pentagon investigation has been effectively complete for months but remains unreleased.
The administration still hasn't publicly accepted responsibility.

BREAKING: Iran's Army, with a total of 420,000 troops, says the majority of its ground force units are now deployed along the country's borders, with a heavy concentration on the Makran coast on the Gulf of Oman, including special forces, rapid-response, mechanized and artillery units, per Fars.
Iran anticipates US ground forces may attempt to invade through the southern coastal cities, coming after Trump said Thursday he is approaching "a major turning point in the war with Iran" and has "a big decision coming up."
The statement follows Iran placing all armed forces on Code 100, its highest alert level, last night.

We are once again facing Trump’s decision on whether or not to resume the war.
Direct attacks on Iran are not the first step being discussed right now, but rather fierce bombings of Yemen accompanied by ground military operations by Saudi-backed forces.
Both the Americans and their allies know that this will bring Iran back into the war, and that is the scenario being evaluated at this moment.
Although Trump has refused the Saudi request to attack Yemen and has also refused to join the naval coalition, global market pressure is making him change his mind in the face of increasingly advanced Houthi attacks.

BREAKING: For the first time in four years, Yemen's Houthis have struck Saudi Arabia's King Khalid International Airport in Riyadh with drones, hitting the main jet fuel tanks, with flames and a massive plume of black smoke over the airport, per Reuters.
