🚨 Your brain makes 100% of its own cholesterol and never once borrows a molecule from your bloodstream. Statins cannot "starve" your brain of cholesterol through your arteries. It is physically impossible. But the internet keeps telling you low LDL causes dementia. "But the data says otherwise." And no, lipophilic statins are not more dangerous to your brain than hydrophilic ones. That myth needs to die too. I am a cardiologist. I have thousands of patients on statins. I check their brains as carefully as I check their arteries. 🩺 Here is the anatomy nobody explains to you: 🔬 Your blood-brain barrier blocks LDL particles, apoB particles, and every lipoprotein circulating in your plasma. 🔬 99% of the lipoproteins inside your brain are HDL particles carrying apoE, not apoA-1. 🔬 Your brain synthesizes every molecule of cholesterol it uses. Brain cholesterol has a half-life of 5 years. 🔬 Lowering your LDL-C or apoB in blood cannot touch brain cholesterol levels because the two compartments never talk to each other. 💓 Here is what the science actually says about statins and dementia: ✅ Meta-analysis, 1.4 million participants: zero studies showed increased dementia risk with statins. Some showed no difference. Others showed protection. ✅ Nature/EurJPC meta-analysis, 5 million participants: statins associated with a 20% decrease in dementia and a 32% risk reduction in Alzheimer's dementia. ✅ Neurology 2024, 4,807 individuals: statins associated with 19% lower Alzheimer's risk. In apoE ε4 carriers, that risk reduction jumped to roughly 40%. ✅ JACC study, ~20,000 patients: statin use showed no association with cognitive decline of any kind. No difference between lipophilic and hydrophilic statins. That myth is dead. ✅ Journal of Clinical Lipidology meta-analysis, over 1 million participants: no significant association between statins and adverse cognitive effects. 10 studies showed reduced dementia incidence. Statins cross the blood-brain barrier and can suppress your brain's own cholesterol synthesis. That is real. That is the one legitimate theoretical concern here, and I take it seriously. A minority of statin users report mild cognitive symptoms. It is on every package insert. I do not dismiss that. I monitor a biomarker called desmosterol in select patients. Low desmosterol may signal over-suppression of brain cholesterol synthesis. This is not standard of care yet. I call it the future of treatment, not current practice. For patients where this concerns me, I have alternatives. Bempedoic acid and PCSK9 inhibitors lower apoB in blood without crossing into brain cholesterol synthesis at all. That matters because the 2024 Lancet dementia report lists high LDL-C as a top modifiable risk factor for dementia, tied with hearing loss. Reducing apoB reduces vascular dementia risk. Undertreating your lipids to "protect your brain" does the opposite of what you think. ❤️ Bottom line: Statins do not cause dementia. Every large study points the other direction. The theoretical risk is over-suppression of brain synthesis in a minority of patients, not blood LDL lowering itself. Get your apoB treated. Ask your cardiologist about desmosterol if you have cognitive concerns on a statin. Do not stop your statin because of an Instagram post. A patient who stays on their statin and treats their apoB aggressively can lower dementia risk over decades, not just heart attack risk. That is the difference between fearing a molecule and understanding it. Why does the myth that "low cholesterol kills your brain" spread faster than the data that says the opposite? #Cardiology #HeartDisease #HeartHealth #CardiovascularHealth #Statins #ApoB #Dementia #LDL #PreventiveCardiology #LifestyleMedicine

That’s because our physician leadership has been happily giving away participation in the process for personal gains such as titles and promotion. I don’t think you can reform it like the cartoon says. We are employees, providers and associates. We have an opportunity to help ourselves by being involved in AI adoption and I am using it daily in my practice and saving at least 2 hours of charting for compliance that has no value for patient care. Feel free to encourage your org to get it. #AIrocks
Reform needed STAT This is not why anyone went into medicine. EHR should help us but not inflict so much pain. We risk losing physicians and we are seeing that a lot more than we should.
