✂ Biopsy forceps... for pulmonary #thrombus removal? When standard balloon angioplasty couldn't break the cycle of recurrent #CTO reocclusion, Nagayoshi et al. went off-label to mechanically remove an organized fibrotic "cap". Read the case: asiaintervention.org/article…

When Is a Coronary Artery “Small”? RCTs suggest DCBs are non-inferior to DES in small vessels. But where’s the cutoff? The magic number for long-term stent patency is an MSA of 5.5 mm², corresponding to a vessel diameter of 2.6 mm. See the sizing chart below. The question of the day: Is a ref. vessel dia. (RVD) of 2.75 mm the tipping point for choosing DES over DCB? 🤔
If 3mm DCB fully yielded without recoil in this young man without heavy calcium then that’s an MLA of 7mm2. It’s adequate.

Thoughts on How to Achieve the Optimal MSA with PCI The suggested cut-off of 2.75 mm in my previous post is to guide the choice between DCBs and DES. It is NOT a suggested target MSA. The goal is to achieve at least 90% of the ref. MSA, as proposed by @SripalBangalore et al. Additional thoughts on how to do this are in my reply.
Interesting way of looking. There are obvious limitations of absolute MSA threshold. A MSA of 5.5 in a proximal artery with ref dia of 4.0 mm, is only 44% area expansion but clearly too big for distal small vessels. In an editorial, we proposed (for LM) the below relative MSA.

Not all apoB-containing particles tell the same story. 🔹 Non–Lp(a) apoB → greater coronary plaque burden 🔹 Greater relative Lp(a) contribution → more vulnerable plaque features New in #EJPC: Balance between apoB and Lp(a) may add another layer to cardiovascular risk assessment and personalized prevention. ow.ly/uL2650ZWCIC @EJPCEiC @AboyansV @SilCastelletti @PTBuketAkinci @PeroneFrancesco @SMosteoru @TimKambic

Cannulas are direct/indispensable interface between patients & #ECMO but may also cause severe side-effects, as thrombosis and hemolysis! Simulation study evaluating impact of drainage cannulas design 🩸 current design reveals no-flow/stagnation areas, which may induce thrombosis, especially if EBF <2LPM 🩸 largest proportion of flow drains through proximal row of holes = risk area for hemostasis distal ⚙️ new geometric design improved pressure loss, but at the expense of pronounced distal no-flow zones ⚙️ geometry of side holes has an important impact: distal growth of hole area results improved flow @ICMexperimental 🔓 rdcu.be/jMXLG1avSyoa

🫀 VESALIUS-CV: Why Does This Trial Matter? Most PCSK9 inhibitor outcomes data has come from secondary prevention: patients who already had an MI, stroke, or major cardiovascular event. VESALIUS-CV asked a different question: can we prevent that first event? More than 12,000 high-risk patients with atherosclerosis or high-risk diabetes, but no prior MI or stroke, were randomized to evolocumab or placebo. Over a median 4.6 years: 📊 12,257 patients enrolled 📊 No prior MI or stroke 📉 25% relative risk reduction in CHD death, MI, or ischemic stroke (3-point MACE) 📉 19% relative risk reduction in the broader 4-component CV endpoint (4-point MACE) This shifts the paradigm: we now have randomized outcomes evidence supporting intensive LDL-C lowering with evolocumab before a first major cardiovascular event in appropriately selected high-risk patients. Prevent the first MI or stroke, not just the next one. 🎥 Watch the full breakdown in our latest reel. Featuring Anurag Mehta, MD, FASPC, Asst. 👉Association of Lp(a) With Coronary Atherosclerosis and CV Events 🤔 Lp(a): perhaps more atherogenic than thrombogenic? 1️⃣ What does SCAPIS cohort tell us? In >28,000 individuals followed for 7.8 years, high Lp(a) was associated with: → More coronary atherosclerosis → Earlier development of plaque → More obstructive disease → ~50% higher risk of coronary events 👆 But here is the key: At a similar plaque burden, high Lp(a) was NOT associated with more events. Mediation analysis suggested that most of the excess risk associated with Lp(a) was explained by greater atherosclerotic burden, with no significant direct effect on events. 2️⃣ Could this help us think about the negative Lp(a)HORIZON trial? Perhaps. If the major clinical effect of Lp(a) is to accelerate atherosclerosis over decades, rather than primarily triggering thrombosis on established plaques, then: Lowering Lp(a) late in the disease course may not rapidly reverse decades of accumulated plaque burden. This does NOT rule out prothrombotic or plaque-vulnerability effects. But it raises an important question after HORIZON: With Lp(a), could timing matter as much as lowering? 🔓🔗 doi.org/10.1016/j.jcmg.2026.… @society_eas @JACCJournals Congrats @EliasBjornson and co-authors
