Covid (@UCSF) Chronicles, Day 848 It’s been a few weeks since my last update – the flow of Covid news & research has slowed a bit. But there’s still plenty of questions that come up, so in this🧵I’ll answer a few that I get asked commonly, including how my wife is doing. (1/25)

I've been open -- exhibitionist?! -- about having a rare tumor. Similarly affected patients will, increasingly, Google our diagnosis & find me. They seldom seek something unorthodox. Most often, they just want to be heard; sometimes, I barely even talk. Listening is medicine.

UK variant – ~50% more infectious & maybe more fatal too – is now surging in Denmark. In @washingtonpost article tinyurl.com/y3aq9rn2, terrific metaphor by Danish prime minister about why redoubling safe behavior is needed, even when things seem OK. That's exponential growth.

I’ve not yet seen report on whether Trump has received dexamethasone (steroids); we know he already got remdesivir (antiviral) & “cocktail” of monoclonal antibodies. But steroids, the only drugs proven to lower Covid mortality, raise a complex set of issues. An explainer. 1/9

Today was an important day for me. My first day back in the OR since my cardiac arrest on May 11th. 7 wks ago, we didn’t know if I’d survive, much less perform eye surgery ever again. 7 wks later, I can do everything I could do pre-arrest, except win an argument with my wife.

It’s Time for a New Kind of EHR per @johnglaser "An overhaul of the electronic health record is overdue. It must go beyond fixing the user interface or improving interoperability. It must address the fundamental problems exposed by the pandemic." ow.ly/S3Vs50A67Cv
I watched a webinar this weekend put on by a medical device company on a gynecologic surgery technique. 12 men spoke, 0 women spoke. GYNECOLOGIC SURGERY. In a field where 50% of us are women and 85% of the trainees are women. So I emailed my feedback to the company ...

I’m not a proceduralist. I don’t know the adrenaline surge of reopening a widowmaker during a STEMI or the satisfaction of a job well done after an R0 resection. But as a medical oncologist telling the patient their cancer has vanished on scan is the best feeling. #MedTwitter

I don’t know who needs to hear this, but a “fast paced, dynamic environment” in a job description is a red flag.

#AllCleanHandsOnDeck This is how we stop COVID-19 and get back to our lives. nejm.org/doi/full/10.1056/NE… Six coordinated, full-court-press steps. Other models just cause a slow, agonizing trench warfare that leaves us in a shambles. HT @CraigABrammer

We are again urging all Marylanders to stay in place at home, practice social distancing and help us stop the spread of this deadly virus. Staying home saves lives.

So please, please, stay the course. The more seriously we isolate and maintain holier-than-thou hygiene, the sooner we can return to our lives and our in-person experiences. Watch the numbers. Listen to the true experts. You are making a difference. Thank you! /end

We take the stress of these moments in stride, and at the end of a long shift go back to our families, hug our kids, walk our dogs & come back the next day recharged. But this #pandemic presents a different set of challenges. Please help us save lives & do the job we love. /END

I am an oncologist. I tell people they have cancer. I tell some people they are dying of cancer. I am used to seeing fear in people’s eyes. I have never seen this much terror, so many grappling with their own mortality. Hope the party in Daytona Beach was worth it.
