On #WorldStrokeDay, we are delighted to announce that CLEAR is supporting Lincolnshire to accelerate improvements to #stroke care. #CLEAR will work closely with front line clinicians and managers from @NHSLincsICB and @ULHT_News to examine the current stroke pathways, address workforce challenges and identify ways to reduce hospital length of stay for stroke patients. Their work will focus on enhancing community-based care - including #rehabilitation services - and streamlining discharge processes – ensuring patients get the right care and support in the right setting. #strokecare #NHStransformation #ActFast

CLEAR Living Well toolkit launched! Today we launched our #CLEARLivingWell toolkit to support PCNs and ICSs to develop targeted and personalised #prevention pathways for those at risk of #cardiovasculardisease (#CVD). Commissioned by @NHSEngland , the toolkit is part of our CLEAR Living Well programme - which has worked with sites across England to design, spread and implement #newmodelsofcare for the prevention of CVD. The toolkit offers a mix of transformation methodology and practical support encompassing seven different models of care developed and tested by front line #clinicians. It is primarily aimed at #PCNs and may also be useful for ICSs that have identified CVD challenges and are looking to develop a system-wide project. CLEARnotes is LIVE! This week we kicked off our #NHS England-funded pilot for #CLEARnotes – our AI-powered clinical notes summary tool. @ELHT_NHS is the first site to go live with the tool – which will support them to transform productivity and release capacity for patient consultations within pre operative assessments. CLEARnotes converts clinician-patient conversations automatically into clinical notes – leveraging AI to produce clear, structured summaries – significantly reducing time spent on paperwork.
