Does adding procalcitonin testing to NEWS2 improve sepsis care? The 7,667-patient #PRONTOTrial found NO change in 3h IV antibiotic initiation (48.4% vs 48.2%), but significantly reduced 28-day mortality from 16.6% to 13.6%! 🧵👇

Why did mortality drop? The procalcitonin algorithm reclassified 24.6% of NEWS2 high-risk patients to low-risk & 33% to medium-risk. The biggest mortality benefit was in the most deprived groups—acting as a 'leveling-up' intervention to streamline care.

Is this algorithm safe? Yes! The 90-day survival benefit persisted (20% mortality vs 23.8% usual care) with NO increase in overall antibiotic exposure (median 5 days in both arms). Safety events and readmissions were statistically identical.
