🚨 A rising PSA after testosterone recovery does NOT necessarily mean metastatic progression. @Soum_Roy_RadOnc delivers once again important data @ASTRO_org !
In 402 men with ~10 years follow-up, faster PSA kinetics strongly predicted progression—but had no association with distant metastasis on cause-specific analysis (HR 1.00).
PSA ≠ metastasis.

Importantly this is modern data that with a spacer gel and SBRT at 2 years 80% of men remain potent.
This is in a prospective RCT with patient reported outcomes.

🤔 urinary function better w NVB sparing

Across 3 prospective studies, the signal for post-prostatectomy SBRT in 5 fx is increasingly consistent: feasible, low toxicity & excellent in-field control. StereoBed randomized data show no PRO/toxicity penalty; POPART/esSBRT show few local failures.

I don’t think this is a controversial statement. The evidence is fairly clear cut now, especially for patients with cribriform pattern. Surgery should be reserved for severe LUTS cases #ASTRO26 #PCSM #SBRT #SABR #Radonc
Controversial statement by @SJFrankMD, informed by the ProtecT study results, for those who still think prostatectomy is the better treatment. #ASTRO26

🎓 Thrilled to have hosted Prof. Alison Tree as a Visiting Professor at @MSKCancerCenter @MSK_RadOnc.
A fantastic day of discussion around prostate cancer, clinical trials, innovation in radiotherapy, and where our field is headed next.
Thank you Alison for sharing your insights and experience with our faculty, trainees, and broader MSK community — and for the many thoughtful conversations throughout the visit.

🧬 Could PSMA PET/CT become more than a staging tool—and actually help us measure treatment response?

PSMA PET/CT Kinetics During Systemic Therapy and Radiotherapy in Hormone-sensitive Prostate Cancer: A Systematic Review
A systematic review of 11 studies evaluated PSMA PET/CT for treatment response assessment in hormone-sensitive #ProstateCancer .
➡️ ADT generally reduced PSMA expression, although an early flare was observed in a subset of lesions, particularly bone metastases.
➡️ Following radiotherapy ☢️ , PSMA reductions appeared greater when ADT was added.
✅ These findings suggest PSMA PET parameters could potentially help personalize treatment monitoring, but their association with long-term oncological outcomes remains to be established.

New in Advances in Radiation Oncology:
MRI-guided whole-gland salvage HDR brachytherapy for radiorecurrent prostate cancer. Prospective phase II, 30 patients, 21 Gy to the whole prostate plus 27 Gy to the MR-defined target over 2 implants, no ADT, median follow-up 72 months.
Key finding: bRFS was 73.0% at 3 years but 24.8% at 5 years (median 44.1 months). Toxicity was low: no acute grade 3-5 events and one late grade 3 (3.3%), with 13% needing temporary catheterization. IPSS and EPIC-50 scores returned to baseline.
Why it matters: this is one of the few prospective salvage HDR datasets with real long-term follow-up and no ADT, so the biochemical curve is not blurred by hormonal suppression. It is the cleanest look we have at what salvage brachytherapy alone actually delivers.
Important caveat: 78% of biochemical failures occurred at the treated target volume, and all 9 patients with persistent disease on post-HDR MRI eventually relapsed. With n=30, 30% Gleason 8-10 at recurrence, and 77% staged before PSMA-PET, this is a signal about dose and patient selection, not a verdict on the modality.
Bottom line: whole-gland salvage HDR is safe and preserves quality of life, but 2 fractions without ADT does not look durable. The next questions are fractionation, dose to the recurrent lesion, and who should be getting systemic therapy alongside salvage.

MAVERICK @SWOG S1827 phase 3 trial Dr. Chad Rustoven presented at @IASLC #WCLC26 comparing prophylactic cranial irradiation (PCI) vs MRI brain with amended primary outcome of cognitive-free failure survival (CFFS).
The study had 68-69% LS-SCLC, and remaining ES-SCLC in the cohort.
MAVERICK identified that MRI brain reduced CFFS, compared to PCI, with NO impact on OS. While brain mets were decreased with PCI, PFS was not significantly different. Significant G3+ toxicity with PCI was observed.
These results FINALLY conclude our suspicions from the Takahashi data (2017) —
⭐️⭐️ PCI has NO clear role in SCLC, in either limited- or extensive-stage, in the MRI era.
