Excited to share a new Call for Papers in Cephalalgia — the official journal of the International Headache Society.
🧠 Special Issue: Managing and Understanding Headache in the Context of Comorbid Conditions
Guest Editors: Faraidoon Haghdoost and Henrik Winther Schytz
Headache disorders such as migraine and tension-type headache rarely exist in isolation. They often sit alongside high blood pressure, dyslipidaemia, depression, anxiety, gastrointestinal disorders, and more. This special issue focuses on integrated, real-world approaches to care — from shared pathophysiology and overlapping risk factors to multidisciplinary management and treatments that address both headache and comorbid conditions.
We welcome:
• Original research and clinical trials
• Systematic reviews and network meta-analyses
• Real-world evidence
• Viewpoints and perspectives
If your work explores headache in the context of comorbidities — cardiovascular, mental health, gastrointestinal, or beyond — we would love to hear from you.
📅 Submission deadline: 31 December 2026
📌 Please note in your cover letter that your manuscript is for the “Headache and Comorbidities” special issue.
Let’s move toward more holistic, patient-centred headache care. Looking forward to your submissions and to advancing this important conversation together.

Chronic morning headaches may signal an underlying health condition. But because morning head pain can be aggravated by a long list of potential culprits, pinpointing the cause can take a little detective work.
Here's what to look out for:

Given that patients may believe that pain always involves observable tissue damage or inflammation, reassurance that pain can also be related to problems with central nervous system (CNS) processing is often needed. Patient education includes messaging that fibromyalgia is chronic and noncurable but manageable and not life threatening, that treatment focuses on calming and resetting a hypersensitive CNS, and that nonpharmacologic self-care strategies play an important role in promoting health.
Pain self-care occurs outside the medical office, in the context of family, friends, and community. Self-management strategies are health-promoting lifestyle changes that can increase positive affect and calm CNS hypersensitivity (seen in figure). Because each strategy involves the formation of a new habit, consistent use of the strategy is needed to produce the desired benefits. Encouragement from the provider team at follow-up visits can help with habit formation and self-care maintenance. A meta-analysis of randomized, controlled trials that examined many different self-care interventions for managing fibromyalgia supports the use of self-care in reducing pain, increasing functioning, and modulating mood over the short and long terms.

ALT An infographic from the Clinical Practice article "Fibromyalgia" by David A. Williams, Ph.D., and Daniel J. Clauw, M.D. It shows the diagnosis and assessment of fibromyalgia. It outlines self-care approaches for managing fibromyalgia, including pain education, sleep hygiene, activity pacing, stress management, social support, physical activity, pleasant activity scheduling, resilience training, dietary modifications, and environmental modifications. Each section provides brief strategies for each approach.

CGRP-targeted therapies have transformed #migraine care, yet many patients remain nonresponders. PACAP-targeted therapies may represent the next frontier in mechanism-based migraine prevention. Check out my recent perspective in @HeadacheJournal #headache

We’re delighted to share that the paper “Transition of headache care from childhood to adulthood: Focusing needs, barriers, and models of care”, a position paper of the IHS Child and Adolescent Committee, has been selected as essential reading by our Editor-in-Chief, Simona Sacco!
Congratulations to all authors and collaborators on this important contribution, which highlights the need for coordinated, patient-centred models of care to support young people transitioning from paediatric to adult headache services.

Cluster Headache Awareness Day is tomorrow 💙
Let’s take a moment to learn how to recognize one of the most painful conditions in medicine, as its diagnosis may be delayed for years.
Earlier recognition means faster, more effective treatment.
Stay tuned and help raise awareness for cluster headache 💪

🧠 Tension-Type Headache (TTH) Awareness — Treatment
Effective management of TTH involves both non-pharmacological and pharmacological approaches tailored to the individual. Non-pharmacological strategies include cognitive behavioral therapy, regular exercise, and a healthy lifestyle, such as adequate sleep and moderation of alcohol and smoking.
When needed, pharmacological treatment can be used. Simple analgesics such as paracetamol or non-steroidal anti-inflammatory drugs are often effective for acute attacks, while preventive medication may be considered for frequent episodic TTH or chronic TTH.

2025 Highlights in medication overuse and medication overuse headache
Wanakorn Rattanawong and Luigi Francesco Iannone
• Medication overuse headache is now recognised as a complex neurological and biobehavioral disorder — not simply “too much medication”
• New 2025 evidence suggests that with CGRP-targeted therapies, stopping acute medication may not always be necessary to achieve improvement
• Mindfulness-based approaches show promise, with brain connectivity changes linked to pain control, emotion regulation, and medication use
• The endocannabinoid system may play an important role, pointing to new biological pathways behind medication overuse
• Long-standing assumptions are being challenged — medication overuse may extend beyond migraine, with emerging data in cluster headache
• Bottom line: MOH is multidimensional, involving biology, behaviour, and psychology — and future care will need personalised, multidisciplinary strategies
