Case of the Day - 236 - It Can be a Slow Burn or a Raging Wildfire
Two patients
First - 57-year-old now presented with multiple areas of progressive consolidation since 2019 with low-grade uptake on a recent PET
Second - 64-year-old presented with rapidly progressive opacities over 40 days with high grade uptake on PET
Both biopsied and showed the same overall diagnosis.

Case of the day - 235 - From NSIP OP to Variant UIP - A 15 Year RA-ILD Journey
65-year-old patient with rheumatoid arthritis (RA) and RA-ILD presented in 2011 with a mixed non-specific interstitial pneumonia (NSIP) / organizing pneumonia (OP) pattern, which over the years has turned into a variant usual interstitial pneumonia (UIP) pattern, with exuberant honeycombing.
The video charts this journey through 10 scans from 2011 to 2016, over 15 years.

ILD Patterns - The UIP Pattern
It is characterized by reticular opacities, subpleural basal predominance and traction bronchiectasis with or without honeycombing. Primary UIP implies IPF, while secondary UIP implies other etiologies
The video describes the UIP pattern, the various signs and their differentials, based on the new guidelines published in Aug 2025.

Managing the Problem of Cognitive Overload
As we grow older, managing cognitive overload is a major issue.
On the one hand, we are bombarded by messages, calls, requests, which our minds can handle only so many of before they go on the fritz. There are some things we can do to manage all these sensory inputs.
On the other hand, is the issue of how to reduce the impact of this cognitive overload, when it does occur despite all our best efforts to reduce all the inputs that can overload our brains. I have some coping mechanisms, but I don't think they are adequate and I wonder if others have better ones.

Lecture - Imaging the Genetic Blueprint - CMR Phenotypes of Inherited Heart Disease
This was my talk at IACI last Saturday. I covered 3 buckets.
1. Known genetic conditions that affect the heart - imaging the heart to quantify and type the abnormality
2. Known genetic risk with equivocal or normal echo - to see whether the heart is normal or not on CMR
3. Signs that point towards a genetic cardiomyopathy in a patient with dilated cardiomyopathy that would initiate genetic testing
a. Hypertrabeculation
b. Ring-like enhancement
c. Biventricular involvement
d. Inferolateral scar
and the concept of a shrinking "idiopathic" cause of dilated cardiomyopathies..

Case of the Day - 234 - Reversible Periosteophyte Friction Fibrosis
64-year-old man with 3-level periosteophyte friction fibrosis on a recent scan, which had regressed compared to 2024, which in turn was new compared to 2019.
The video goes through the case and then the existing, extremely sparse literature on this subject and tries to answer these questions
1. Why does periosteophyte friction fibrosis occur?
2. Can it reverse?

Case of the Day - 233 - Rapidly Growing Mediastinal Mass
22-year-old with a prevascular space mediastinal mass extending into the visceral space and markedly increasing in size over 9 days with pleural and pericardial involvement
Thoughts?
1. Lymphoma
2. Germ cell tumor
I biopsied it and we got a diagnosis, a specific histology, which apparently should have been the first diagnosis in my head...but I wasn't aware of this till I read it up.

Sion Bridge is a small but critical railway bridge in Mumbai
It was 112 years old.
The govt realized after 112 years old that it is 112 years old and needs to be replaced.
They said it would take 2 years to replace a 60 mtr bridge.
When the Bridge was there it took me 45 mins to get to office and 1 hour to get back.
Since the bridge is demolished, I spend one and a half hours to reach office and an hour and 45 minutes to get back.
That is one and half additional hours in commute, every day, for the last two years
Which makes it 7.5 hours a week
Or 390 extra hours a year.
Which becomes 780 extra hours over the last two years
That's a 32 extra days spent on commute over the last two years.
Or one whole month.
Just on commute
This is extra or over and above the 32 days I anyway used to spend on commute in two years.
And this is just me.
And there are thousands like me
One small bridge. One massive impact.
Hope the govt completes it soon.
That's all we ask

Improving Your Cardiovascular Fitness...One MET at a Time
If you do nothing at all, walking 4000 steps a day is where to begin, and that alone will change your life.
But if you already walk every day, more walking will not take you any further. The number that decides how long and how well you live from here is the VO2 max.
In this piece I explain what it is, what you should be aiming for at your age, and exactly what to do to move it.
