Amazon Web Services (AWS) experienced multiple major failures yesterday, taking down operations as big as Venmo. I'm very proud to say Brass Nine's clients experienced no outage whatsoever, because of reliability work we did 2010-2011.

We took over a C# site from another vendor. In the code, the previous vendor left in... root user, password, and IP for their SQL server.
I don't think this is a rare occurrence, it's just rare one vendor sees another's code.
