Plaque buildup in the carotid arteries is a major & modifiable risk factor for ischemic stroke. This study examined whether AI-enhanced handheld ultrasound could improve carotid plaque detection & stenosis assessment in community-based #PrimaryCare screening ๐Ÿงต LINK BELOW

Results: The AI-enhanced images showed 94.8% of plaques compared with 87.6% on the standard handheld ultrasound images. The AI-enhanced images correctly flagged 63.2% of plaques that looked unstable, up from 47.4%, and correctly ruled out stable-appearing plaques about 96% of the time. annfammed.org/content/24/5/4โ€ฆ

#FamilyMedicine offices can be constrained by the availability of clinical support staff. Medical assistants (MAs) are responsible for performing a variety of tasks in many offices. When trying to coordinate multiple MAs in a large multi-physician practice, identifying an optimal allocation of tasks can be challenging, and suboptimal MA task allocation can cause bottlenecks, prolonged visit times, and delays in care.

In response, the practice began trialing a new arrangement which involves matching each medical assistant to a single physician or a pair depending on patient load rather than sharing them across a pod of physicians. The practice has seen an improvement in workflow and satisfaction among physicians and assistants has improved. ๐Ÿ”—annfammed.org/content/24/5/4โ€ฆ

A study of a Teamlet care model in Singapore found that reorganizing large polyclinics into small teams for chronic disease care can improve care processes & reduce high-cost health care utilization, with a shift of healthcare utilization towards #PrimaryCare ๐Ÿงต [Link Below]

The teamlet model was associated with decreased overall health care costs for people with chronic disease over 6 years. The savings came mainly from fewer hospital stays, and to a lesser extent, decreased specialist outpatient visits, and fewer emergency department visits over six years. #PrimaryCare costs rose due to patients receiving more comprehensive care and visiting the polyclinics more often.

A New @AssnDeptsFamMed Resource: Making the Evidence-Based Business Case for #FamilyMedicine to Institutional Leadership. To support conversations with decision-makers, ADFM has developed a set of resources that help department leaders articulate the strategic value of Family Medicine in different settings. These resources are designed to assist chairs and faculty leaders in preparing for discussions with medical school deans, health system executives, and other institutional stakeholders.

Over a 12-month period beginning in March 2023, 801 HVVs were completed and, in comparison to a demographically similar control group within the same risk pool, the HVV patient population saw a 9.2% reduction in per-member-per-year costs, translating to $17.6 million in global savings. annfammed.org/content/24/5/4โ€ฆ

Targeting of rising-risk, as opposed to current high-cost claimants may be an effective strategy in mitigating health care expenditures within underserved, urban primary care environments. In conclusion, the model led to reduced care utilization, improved chronic disease management, and lower global costs within our rising-risk Medicaid managed care population. annfammed.org/content/24/5/4โ€ฆ
