Great foregut session on PEH repair @ #ACSCC2025. @houghton_dr gave a great talk on difficult cruroplasty. 1. Use your tools (⬇️pneumo, mesh, relaxing incision…) 2. Experience matters - should be done in high volume centers 3. Defect shape may predict success of closure

While we hope to not create or encounter fistulas 😥😳😟, @SLBachman is helping us be prepared to manage that disaster. Big take homes - don’t hold off on enteral feeds until surgery & educate your patient so they can manage expectation. This will be a long road. #ACSCC2025
