I pray for the strength to accept whatever outcome may be presented to me after the boards. I know that I prepared for this, and I prayed harder. No regret shall haunt me, for I have given my all.

Bromocriptine is not just a prolactin drug.
It is a morning calming agent.
That is not branding. That is the mechanism in type 2 DM.
Most students meet bromocriptine as:
D2 agonist β β prolactin β pituitary adenoma / Parkinsonism.
Then the exam throws βalso used in type 2 diabetesβ and the room goes silent.
The useful version is this.
Seasonal animals do something humans call disease.
Migratory birds and hibernating mammals flip, on a clock, from insulin-sensitive to insulin-resistant. Muscle and liver stop listening. Hepatic glucose output rises. Fat is stored. That state is a survival setting for a season when food will disappear.
The switch sits in the hypothalamus, not in the pancreas.
In the insulin-sensitive season, dopaminergic tone at the circadian clock (SCN) peaks at the start of the active day. In the insulin-resistant season, that morning dopamine peak is missing. Sympathetic drive stays high. The periphery then looks like type 2 diabetes: insulin resistance, more hepatic glucose, more lipolysis.
Type 2 diabetes looks, neurochemically, like an animal that never left winter.
Quick-release bromocriptine was built around that observation.
It is a sympatholytic D2 agonist given within 2 hours of waking. The point is not to raise insulin. The point is to put back a morning dopamine signal the hypothalamus is not making, so sympathetic tone falls, and post-meal hepatic glucose production falls with it.
That is why the timing is not optional. Give it at the wrong phase of the day in animals and the metabolic effect collapses. The drug is trying to copy a circadian peak, not occupy a receptor all day.
If this shows up in a paper:
β’ T2DM formulation studied: bromocriptine-QR (Cycloset), FDA-approved 2009
β’ Start 0.8 mg, titrate, max 4.6 mg, once daily, morning only
β’ Glucose comes down without a rise in plasma insulin
β’ Same molecule as the old prolactin / Parkinson drug. Different dose, different clock, different intent
If a question asks why bromocriptine for diabetes is taken in the morning, the answer is not convenience.
The brain is supposed to say βcalm downβ at dawn.
In T2DM that line is missing.
Bromocriptine is the line.
Educational purpose only. Not clinical advice.
