A misplaced mandibular nerve block can temporarily paralyze the facial muscles.
During an inferior alveolar nerve block (IANB), excessively posterior needle placement may result in local anesthetic deposition within the parotid gland, where the facial nerve (CN VII) passes through. The anesthetic can temporarily block the facial nerve, causing ipsilateral facial paralysis, including inability to close the eye and deviation of the mouth toward the unaffected side.
Clinical significance: This is a recognized complication of mandibular block anesthesia, but it is distinct from classic Bell’s palsy, which is usually idiopathic. The paralysis generally resolves as the anesthetic wears off. The affected eye must be protected from exposure and drying until normal eyelid closure returns.
Prevention: Identify the anatomical landmarks carefully and maintain the correct needle position and depth to deposit the anesthetic near the mandibular foramen, slightly superior to the lingula—not within the parotid gland.
Key learning point: If facial paralysis develops immediately after an inferior alveolar nerve block, suspect inadvertent facial nerve anesthesia, but arrange prompt assessment to exclude other causes of acute facial weakness.

How Should We Determine the Incisal Guidance Angle in Complete Dentures?
Unlike natural teeth, incisal guidance cannot be measured directly after tooth loss. It must be determined by the clinician based on the condition of the residual ridges and the stability of the denture bases.
The key principle:
* Good ridge conditions and denture stability: Steeper incisal guidance can improve denture function.
* Poor ridge conditions and denture stability: Flatter incisal guidance helps minimize denture displacement.
As a general starting point for moderately favorable residual ridges, a sagittal incisal guidance angle of 10° and a lateral incisal guidance angle of 10° have been suggested for stable occlusal adjustment.
The goal is not simply to create guidance, but to balance function with denture stability.

As you adjust the occlusion of more dentures intraorally, you will begin to recognize that each patient has a unique direction of lateral movement, sometimes differing between the right and left sides.
Proper occlusal adjustment prevents denture displacement during mastication, maintains stability, and helps minimize mucosal inflammation, pain, and long-term residual ridge resorption.
Inadequate occlusal adjustment can lead to rapid ridge resorption, loss of denture fit, mucosal inflammation, and pain.

Lateral Occlusal Interferences
Lateral deviations during habitual opening and closing movements, combined with acquired chewing habits, can cause occlusal interferences even without food in the mouth, disturbing occlusal balance.
Selective grinding is therefore essential.
On the working side, the BULL rule is applied:
* BU: Buccal inclines of maxillary buccal cusps
* LL: Lingual inclines of mandibular lingual cusps
On the balancing side, the LU inclines of maxillary lingual cusps are adjusted.
Proper adjustment helps establish balanced and harmonious occlusion in complete dentures.
