Dear Prof - can I ask you to spend a couple of your valuable hours looking over the latest offering from the National Screening Committee on Cardiac Screening to prevent sudden cardiac death in the young. I know this isn’t your field but this process desperately needs scrutiny from independent clinical academics even to examine its rigour. The consultation has now closed, but even when it opened the committee said it wasn’t minded to change its previous decision.
The document to support this standpoint is riddled with illogical and circular arguments and misses out numerous recent studies and international guidelines.
In particular the screening committee does list in evidence it will consider the ESC or AHA or FIFA position statements of cardiac screening in youth sports. There is no consideration of the JACC 2026 article reviewing outcomes of CRYS 10 year screening of more than 100000 voulnteers. The committe states that there is no evidence that screening can reduce sudden cardiac deaths while ignoring the ESC document on Ventricular arrhythmias which are evidence based consensus guidelines on managing conditions associated with SCD which the screening program would aim to identify the committee says that a screening program may make people anxious and put them off sport but doesn’t offer any evidence of this. They argue there isn’t infrastructure to support screening but ignore the fact this is the case for most screening programs - the workforce and infrastructure have to be provided for the screening to happen. They attempt to equate Israeli screening results with Italian while not acknowledging the Israeli results are based on newspaper reports of deaths in 2 newspapers and the Italian results are based on a comprehensive longstanding registry of all child and juvenile deaths. They draw on concerns from the US about the cost to families of follow up care after screening but don’t acknowledge this is less of an issue here due to the NHS. They don’t even refer to Japan and Taiwan which are the only 2 countries in the world who screen all their youth. Some serious clinical academics need to look at the “work” the committee have done here and at least call out their lack of rigour.

RUTF - Ready-to-Use Therapeutic Food - is ultra-processed and therefore intrinsically dangerous and unhealthy.
Should we be worried that @UNICEF is one of the biggest distributors of RUTF?

The public may be shocked to learn that there is NO MANDATORY TRAINING for pharmacists in 🏴to become diagnosticians as part of the Pharmacy First scheme.
None.
Nada.
Zero.
Let that sink in!
How @rcgp & @NHSE_WTE let that happen I’ll never understand.

My take on UPFs continues to be:
- there is a need for understanding attributes of foods that influence health beyond classical nutrients, creating metrics around them, and incorporating them into dietary guidance, labeling and regulations. NOVA is a sledgehammer approach to this that has appeal because its an existential cudgel to the industry but is generally unserious as a research tool, let alone a regulatory science tool.
Unfortunately, submitting a grant to NIH to understand the physical and sensory properties of food that impact health isn't sexy enough to land a grant. We've also gutted the research infrastructure needed to do this work, which the pre-2010s research environment wasn't ideal for as is. We continue to splice and dice FFQs in a million ways that won't and can't advance nutrition science and the UPF convo is mostly a swan song for a field charged with addressing major public health crises that its ill-funded to address.
I love to start discourse, thank u @sapinker! And as the super sharp @KCKlatt has told me, the NOVA definition of ultra-processed foods has nothing to do with processing.

I've always thought that the campaign against "ultra-processed" foods was mindless. What the heck does that even mean: the more things you do to food, the less healthy it is? New RCTs and analyses confirm that the stigma is meaningless (it's calories, fiber, & other causes, not "ultraprocessing") and even harmful: it encourages RFKj's MAHA quackery. I suspect that food researchers are subject to class bias: food that poor people like and can afford must be unhealthy. vox.com/future-perfect/49404…

In honour of today's British Grand Prix, I am posting one of my favourite sketches from A Bit of Fry & Laurie, which not only stars the terrific two, but was written by them as well.
Stephen Fry was supposed to mime or pull the punch at the end, but he accidentally connected. You can see by his reaction immediately after, as he looks surprised and slightly concerned. Cracking right hook!
T'is lights out, and away they jolly well go!

If you aren't willing to learn and use these phrases
then please do not ask me questions about my decisions based on science.
1. They aren't really questions
2. You see my answers as arguing points, not information.
3. You aren't seeking info; you're making a point.

I am having a drink this evening with a friend in a Chiswick pub. Two policemen have just come into the pub and asked me to step outside. I have stepped outside and they have threatened me because I tweeted about a councillor banning seating outside pubs in Chiswick. They admit on video (watch it!) that I did not break the law at all. They came to threaten me. To warn me off tweeting about councillors and the council. This is modern Britain. This is the police state. Please, please, please watch this video. It does involve me using very bad language, but this has got to be seen. Police coming out to threaten someone who hasn’t committed a crime. I’m fuming.
