It’s funny…cardioversion was ok when it was done in the two positive sham control studies, but people hate it when the trial was null

I’m sorry but how does @escardio give MRA (without differentiating sMRA from nsMRA) a Class I for HFpEF?
Where is the evidence for steroidal MRA?
And even if we base on nsMRA, we have one study (FINEARTS-HF) and we somehow end up with a Class I?
Make it make sense.

Conducting a RCT is challenging. Conducting an independent RCT is very challenging. Conducting a RCT that influences evidence and practice is highly rewarding. Congratulations to the investigators of the #CLOROTIC and #ADVOR trials

Now for the next big question…. Is scar the arbiter of ICD benefit in non ischaemic CM with EF<35%?
BRITISH trial will tell us … @drflett @bhf
Among patients with ischemic or nonischemic cardiomyopathy, left ventricular ejection fraction of 36% to 50%, and myocardial scar on #CardiovascularMagneticResonance, implantation of an implantable cardioverter-defibrillator did not significantly reduce sudden cardiac death or hemodynamically significant ventricular arrhythmia vs an implantable loop recorder. Rates of all-cause mortality, cardiovascular mortality, and heart failure–related hospitalization were also similar between groups. #ESCCongress ja.ma/4gvoZHO

ESC 2026 starts today in Munich. My main trials to watch: DANHEART, TRIC-I-HF, STAREE, PVI-SHAM-AF. And, of course, there will be plenty more worth discussing. Looking forward to the results and the details behind them. #ESC2026

The 2026 iCARDIO Alliance Global Implementation Guidelines on Heart Failure are out.
What makes them different: not only what should we do? but how can we actually implement it across health systems with very different resources and access.

Your morning coffee either raises your LDL cholesterol or does nothing to it, and the single variable that decides is whether you poured it through a paper filter. That's one of the odder findings in the new AHA scientific statement on caffeine and the heart. 🧵

Dr. Scott Berry and Dr. Kert Viele examine the facts behind bias in early stopping of clinical trials in the latest episode of "In the Interim…".
They clarify the mathematical definition of bias, demonstrate how simulations quantify and adjust for it, and directly address misconceptions fueled by the Bassler et al (STOPIT-2) paper.
✅️ Regulatory agencies routinely require explicit bias calculations
✅️ Most early stopping bias is demonstrably small and manageable
✅️ Published critiques often conflate post-hoc selection with bias
Listen now to understand why "bias" doesn't mean "don't do it." berryconsultants.com/resourc…

Exercise-based cardiac rehabilitation for atrial fibrillation: Cochrane systematic review, meta-analysis, meta-regression and trial sequential analysis - New paper and podcast.
Professor James Rudd (@jhfrudd) chats with Dr Ben Buckley (@BuckleyBenjamin) from John Moores University. They discuss his Cochrane systematic review on the use of cardiac rehabilitation in patients with atrial fibrillation, and whether it might lead to a change in the guidelines.

The Second Universal Definition of Heart Failure has just been published.
🧵 A short thread on the key changes, how it compares with the first Universal Definition and the 2023 ESC guidelines, and one controversial decision likely to spark debate. 1/10
This is probably the key tension.
Biologically, rigid EF cutoffs are artificial.
Practically, abandoning cutoffs may make classification less convenient unless future guidelines provide clear decision thresholds for treatment, devices, and trial eligibility. 9/10
