#Weekend_review #menin_inhibitor #leusm 𧡠Menin inhibitors in AML: biology β approvals β combinations β resistance
1/9
Menin inhibition has rapidly moved from a biological concept to an established therapeutic strategy in AML.
Why does it work?
KMT2A-r and NPM1-mut AML are genetically distinctβbut both converge on the HOXA/MEIS1 transcriptional program, which maintains leukemic stemness and blocks differentiation.
Menin helps sustain this program.
Menin interacts with KMT2A and helps maintain transcription of genes including HOXA9 and MEIS1.
Blocking the meninβKMT2A interaction suppresses this leukemic program and promotes myeloid differentiation.
Importantly, mutant NPM1 also creates a transcriptional state dependent on this axis:
KMT2A-r β MENIN/HOX-MEIS β NPM1-mut
Think of menin inhibition fundamentally as a differentiation strategy, not simply cytotoxic therapy.

IMPORTANT: The TRS refresh is expected around the 19th August 2026. If you havent already, download your CURRENT TRS. Rather stupidly, BSA dont allow you to access your OLD TRS on the portal.. and HUNDREDS OF THOUSANDS didnt have a TRS last year, so if thats you this year, your TRS can dissapear! *Always* download it, check it and store it (email yourself) each year to look for obvious errors

We've written to NHS.net Connect about changes due on 17 August.
Doctors must not lose essential access when changing posts. This could affect rotational doctors, locums and doctors with multiple NHS roles.
We have asked for a pause while safeguards are agreed.

The most contentious issue of the latest NHSE guidance is the explicit mention about looking back at an episode of care one was previously involved in to see how things developed for professional "curiosity".
Curiosity is a very poor word to use for multiple reasons not least because professional curiosity is the bedrock of holistic medicine and their phrasing comes across as a loaded word to diminish care, reflection, and professional development.
Many have argued that "it is just common sense" however one of the problems that is inextricable from the concept of common sense is the trust placed in those who judge how the language used in guidance is interpreted.
If no one trusts NHSE or regulators then everyone will naturally lurch to being overly risk-averse which will create inefficiencies, unforced errors, and damage morale.
It should be uncontroversial to say that those who were never involved in patient care and yet go on to access patient information should be disciplined, if not sacked.
But those who were previously involved in episodes of care should be given the professional latitude to follow up.
Surely the professional who remembers a patient and cares enough about them to follow up, reflect, and learn is a legitimate and valid reason.
@NHSEngland must come out and clarify this urgently or they risk either undermining professional development or some newfound unintended consequence like institutions getting patients to sign away all rights and protections in new waivers (hello website cookie disclaimers)

Just reading over the new rules on IG, as circulated in letter from Jim Mackey.
It is entirely appropriate to reiterate that patient records should be ONLY accessed for work-relate purposes. But the new rules are draconian and restrict the types of reasons doctors often do

Nigel Farage is a grifter. He always has been and he's continuing to grift.
He knew he was heading towards a byelection anyway so has pulled the trigger early.
The people Vs the establishment?! Reform are literally part of the establishment.

History taking is an essential clinical skill, and it is a SKILL. Patients narratives change according to time, place, person, and context as well as who is asking the questions and how. The notion that you can have a single authorised version that is immutably 'true' is absurd.
Replying to @jamesmurray_ldn
You don't even have working computers, get the basics right first.
Also how clinically divorced are you to suggest that repeated history taking is a negative.
It can be essential to ensure that nothing has been missed, each specialist team will ask similar questions but with different thought processes.
Having IT systems fit for the 21st century is essential. Why don't you do your job rather than commenting on whether something is unnecessary as it undermines patient confidence on what can be an essential part of history taking.

Medicine is a distinct & elite profession & Iβm getting tired of people trying pretend that thereβs a shortcut to the years of hard work it takes to become a doctor
Tiered rotas are nothing more than gaslighting patients into worse care
Iβm glad itβs being called out publicly
Every senior consultant was, at some point, a resident doctor. You cannot run a health service without resident doctors, who are the consultants of tomorrow. That this self-evident truth needs to be repeatedly stated speaks only to highlight the bizarre world we are living in.
