Our trial is out in NEJM. SWITCH-SWEDEHEART randomised 7 Swedish regions, not individual patients, to a default prasugrel or a default ticagrelor policy in 17,095 patients with acute coronary syndrome undergoing PCI.
At 1 year, prasugrel policy vs ticagrelor policy:
MACE 11.1% vs 11.8%, adjusted OR 0.90 (95% CI 0.77 to 1.06)
Major bleeding 4.2% vs 4.4%, adjusted OR 0.80 (0.64 to 0.99)
A default prasugrel policy did not reduce ischaemic events. The estimate is a policy effect: the recommended agent was dispensed to 61% under the prasugrel policy and 76% under the ticagrelor policy, which attenuates the contrast.
The trial ran inside SWEDEHEART for about EUR 25,000, and the shift to generic prasugrel saved considerably more than that while it ran. Randomised post-approval evaluation does not have to be expensive.
