In challenging anatomy, to identify the ejaculatory duct orifice, we transrectally puncture the seminal vesicle and inject indigo carmine. Then, with a flexible endoscope positioned at the verumontanum, we look for dye egress. Sometimes it is quite helpful!

Nightmares of RIRS. Respiratory movement is risky and cause significant bleeding. Procedure aborted and fortunately it went well no significant hematuria, hematoma or late vascular complication. General anesthesia with controlled apnea is very helpful to avoid such nightmare
