1/7 85-86% of IBD patients in remission report food intolerances, versus 33% of controls. A Gastroenterology paper argues inflammation causes this: colitis depletes the bacteria that digest dairy proteins, and colonic mast cells do the rest. gastrojournal.org/article/S0โ€ฆ

Part three of the IBD pipeline series. Not just whatโ€™s coming... but when. Iโ€™ve mapped every phase 3 trial in UC and Crohnโ€™s against ClinicalTrials.gov. Start date to primary completion. Company and NCT number for each. 16 phase 3 studies still to complete. 12 of them are due in 2028. All three anti-TL1As (the first wave - there are others stacking up behind). Oral IL-23R (icotrokinra) in UC and Crohnโ€™s. Co-formulated gus + golimumab. Mirikizumab plus tirzepatide. Two have already delivered (*press release only as of today). Obefazimod: positive phase 3 in UC, US filing planned for Q4. Tulisokibart: positive UC induction study in June. Next up... afimkibart in UC, due to complete about now. Most of these are recruiting today. Every patient we offer a trial brings 2028 closer.

Part four of the IBD pipeline series - phase 2. 22 programmes to watch in UC and Crohn's. Start to primary completion, company and NCT number for each. 12 of them are due to complete by the end of 2027. And some of it lands NEXT WEEK at UEG in Barcelona. Sunday: Spyre's three long-acting antibodies in UC Monday: first data for Gilead's oral ฮฑ4ฮฒ7 (emvistegrast)... and risankizumab plus an anti-ฮฑ4ฮฒ7 in Crohn's (ABBV-466). Both late-breakers. Tuesday: RIPK2 (OD-001), PSGL-1 (ALTB-268) and an anti-fibrotic for stricturing Crohn's (ontunisertib) Three have passed primary completion with nothing out yet. Zasocitinib in UC (Takeda says data by March 2027). Eltrekibart (Lilly). AZD7798 (AstraZeneca). [1 of 3]

Drink your coffee black (no additives) if you want the full health benefits. 2-3 cups per day has been associated with a 25% reduction in all-cause mortality. That benefit goes away when cream and/or sugar are added. I think this happens for a few reasons: โ€ข Dairy proteins can bind to coffeeโ€™s antioxidants and polyphenols and slow their absorption. โ€ข Heavy cream, butter, and MCT-heavy coffee donโ€™t necessarily block coffee polyphenols, but they can add substantial saturated fat and calories. For someone watching ApoB or LDL-C, this can work against the very cardiovascular benefits theyโ€™re hoping to get from coffee.
