The Dutch Health Council has just defended the Dutch Protocol for youth gender medicine, and the international community needs to pay attention.
The Netherlands created and exported this model. It is now trying to present itself as the careful exception, while avoiding the deeper scientific and ethical questions being asked in the UK, Finland, Sweden, and elsewhere.
Last week, an unusually broad Dutch coalition sounded the alarm in a national newspaper. It included Pieter Omtzigt, one of the Netherlands’ most prominent former MPs and institutional watchdogs; Dorine Sellenraad, a clinical psychologist formerly affiliated with a gender clinic; emeritus professor Armand Girbes; former politicians; clinicians; writers; people with lived experience; and myself, a former long-time staff member at COC Netherlands, the Dutch LGBT organisation.
Our concern was simple: the Netherlands must finally ask whether this model of youth gender medicine is scientifically and ethically justified.
Now the Health Council of the Netherlands has issued its advice.
And after thirty years of offering a medical pathway to minors that can involve infertility, loss of sexual function, effects on bone development, and lifelong dependence on hormones, the central conclusion is essentially: the Dutch system is careful...
That is a remarkably weak answer to a much more serious question.
The issue is not whether the process is tidy, professionally organised, or takes longer than in some other countries. The real question is whether the treatment model itself is justified in the first place.
That is the elephant in the room, and much of the Dutch media is now refusing to confront it.
Instead, the international criticism is being reframed as a misunderstanding. One Dutch media report even repeated this claim almost verbatim:
“The Health Council also wants to clear up a misunderstanding. In the United Kingdom, the ‘real’ Dutch method was never applied. Whereas British children with doubts about their gender identity could receive puberty blockers after only a few consultations, in the Netherlands this takes years.”
This is deeply misleading.
The Cass Review did not merely criticise poor implementation in the UK. It asked the fundamental questions the Dutch debate is still avoiding:
What is the evidence?
Who truly benefits?
What are the long-term outcomes?
What happens to young people who are not medicalised?
And are the assumptions behind this model of care actually sound?
The Dutch Health Council has not seriously answered these questions. It has assessed the process around the Dutch model, but it has not truly examined whether the underlying model itself is justified.
Likewise, Riittakerttu Kaltiala and colleagues in Finland, using the best available national data, have shown that many of these young people present with serious co-occurring psychiatric difficulties and that medical transition does not make those underlying problems disappear.
Dutch media and institutions are closing ranks around the status quo. External correction may now be the only way to break through this framing.
If you are a scientist, clinician, journalist or policymaker outside the Netherlands: please pay attention.
Read the Dutch Health Council’s advice. Share what is happening. Alert colleagues in your country. Ask whether the Council has truly answered the central questions, or merely defended the process around a model whose evidence base remains deeply contested.

The Netherlands exported the Dutch Protocol. It must not now be allowed to evade international scrutiny by saying: “we do it carefully.”
Please share! And help ensure the Health Council of the Netherlands is challenged by those who understand the international evidence.
What a disaster. It makes the Netherlands look conservatieve compared to this French fanatisme #euthanasia
There will be carnage amongst hospices and care homes in the UK if the #assistedsuicide bill passes.
In France, where their bill has gone further: 'The Little Sisters of the Poor, who care for 2,500 elderly residents in some thirty homes across France, have categorically refused to "take lives" in their facilities, even if it means closing some homes or pursuing multiple legal challenges.'

Natuurlijk is iedereen tegen geweld, dwang, elektrische schokken of vernedering.

Moge zij rusten in vrede.

If there is precisely one thing you watch today, make it this. French Senator Claude Malhuret. A microphone. And the most magnificently savage dismantling of the Trump administration ever delivered in a language they almost certainly don’t speak.
He covers Iran. He covers corruption. He covers the kind of staggering, industrial-scale incompetence that would get you fired from managing a car park. And he does it with the calm, unhurried certainty of a man who has read every page of the indictment and found it, if anything, worse than expected.
France has never pretended to like these people. But this is contempt elevated to an art form. The kind of refined, aristocratic disdain that takes centuries of civilization to produce and approximately ninety seconds to deploy.
Malhuret sounds like he is four seconds from the button. Not out of panic. Out of sheer, exhausted disgust.
Honestly? Understandable.
Watch it. Share it. The adults are speaking.
