4 million people illegally bought drugs abroad not because they chose a riskier supply chain — because payers engineered their coverage to shift costs onto patients. Closing the border solves nothing if that stays untouched. link.tiicann.org/0tdziut #PBMReform
Helen Branswell, STAT’s longtime infectious diseases reporter, has died at 69. For a decade she made the science of outbreaks accessible and timely for readers who needed it - clinicians, advocates, and patients alike. A real loss. Our thoughts are with those closest to her.
Insurers posted billions in Q2 2026 profits while six are exiting ACA Marketplaces and leaving 650,000 people to find new plans. For people with HIV or hep C, losing coverage means losing treatment. Three shocks, one gap. link.tiicann.org/dyd808q

Early results show people losing coverage due to early implementation of a Medicaid work requirement in Nebraska, with signs that they're being caught up in administrative barriers.
Among new Medicaid applicants subject to Nebraska's work requirements, 86% met the requirements or qualified for an exemption — while 14% were denied.

Over the past year, #HIV funding cuts have threatened the progress that PEPFAR (The U.S. President's Emergency Plan for AIDS Relief) helped create in 2003.
Now, "at least 1,714 HIV service sites around the world, including public health facilities, mobile clinics, drop-in centers, and specialized HIV facilities, closed entirely after terminated or delayed PEPFAR payments. Almost no organizations were able to replace lost funding from other sources."

We recently commissioned a poll finding that a large majority of Californians want large scale health insurance reform, including holding conglomerates accountable for the role they play in rising costs, especially through vertical integration. The spillover effects on the #340B program are front and center as well, with 91% of voters agreeing the program needs accountability and transparency.

Things @TIICANN has been talking about for years. PE owned hospitals seek to convert to non-profits *because* of 340B. ->
A private equity owned hospital chain is converting to a nonprofit. On its face, it sounds like a good deal.
But who actually benefits? Quorum's hospitals will get loaded with 30% more debt while Quorum's lenders are almost made whole.

Per @abettel at Axios: the Labor Dept says it will enforce mental health parity, targeting the network and prior-auth gaps that keep behavioral and SUD coverage below physical health.
Right direction. The test: does December's rule reach network adequacy and reimbursement with teeth, or is it parity on paper?
Coverage is only half the access problem. The other half is a system that defaults to enforcement over treatment, and parity covers SUD care too.
Exhibit A, via @KFFHealthNews, @aneripattani and @thecurrentlaf: Louisiana routed 20% of its opioid settlement to sheriffs, who spent it on Flock cams and other inappropriate equipment.
The through-line is we know what works: network adequacy, trauma-informed care, treatment over cages. We keep choosing to put it out of reach.
