The Weight represents Simon's survivor's guilt, his traumatic memories of the accident, and feeling he's "carrying" the victims with him. The 3 IV drip stands piercing its chest in the boss fight symbolize the 3 victims whom he got blood transfusions and a heart transplant from. Details about those victims are mentioned during the blood transfusion puzzle in the chapter "Many into one". The chapter's title also seems to be a reference to the Weight.
