Our rapid diuresis team is a geolocated unit of HF patients. Diuresis starts in ER & continues 24/7 as needed. A patient-centered multidisciplinary team makes a plan personalized to patient goals/psychosocial situations. Discharge includes follow-up/outpatient diuresis plan.
ALT On 3/24/23, a multidisciplinary group met to re-imagine inpatient care of heart failure patients with the goal to reduce length of stay all the while maintaining top quality care. The team was comprised of AHF physicians, CVM rounding nurses, HF nurse navigators, APPs, nursing, palliative care, mission control, quality improvement and leadership.
