I’m 56, a practicing physician, and still training like it matters. This account is about healthy, successful aging and longevity — the kind that keeps strength, energy, and independence high as the decades add up. Evidence over hype. Habits you can actually keep. Science you can use. If that’s the lane you want, follow along. Bigger things coming.

And over time, the power of those ‘treat’ foods lessens. GLP drugs and the like quiet the noise so you can develop the habits that quiet the nose yourself.
Metabolically heathy people can have very flexible diets! The better shape you are in the less careful one can be with monitoring occasional “treat” foods. Thing is most people who can eat them, simply don’t. They tend to be very routine

I have had hypertension since I was in my 20's. First time I met my current physician, I ask him if he wanted to raise my blood pressure meds. Response? "Let me see, your blood pressure was "140 over too fat, right? Let's see you lose ten pounds and recheck". Simply to spite her, I lost the weight, and lost the medications. Still my doctor 30 years later!! Medicine needs more hard passes!
Of those I know that were clinically obese on prescribed GLP, I saw a friend with a protein shake and gave her kudos. Her response was that her doctor said he would end the prescription if she didn’t increase her protein intake. I have respect for that doctor.

Gym bros, just because you're lean, muscular, and metabolically healthy and can eat whatever you want while keeping calories in check doesn't mean someone with obesity, fatty liver, prediabetes, or type 2 diabetes, can do the same. You're confusing being metabolically healthy with understanding metabolic disease.

Data also show the metabolic benefits also persisted in the withdrawal group. Lipids, liver function, kidney function markers stay improved. You can bad mouth Big Pharma all you want but if their ‘life long’ drugs improve my life over what I could do on my own, then count me as a Big Pharma advocate.
Stopping tirzepatide is the part of the GLP-1 story that doesn’t fit into a before-and-after photo. The clearest evidence comes from SURMOUNT-4 (Aronne et al., JAMA, 2024), a randomized withdrawal trial funded by Eli Lilly. Stage one: 783 adults with obesity or overweight, without diabetes, received tirzepatide for 36 weeks. The 670 participants who reached randomization had lost an average of 20.9% of their starting body weight. Stage two: Participants were randomized to continue tirzepatide or switch to placebo for another 52 weeks. The results: Continued tirzepatide: another 5.5% weight reduction. Switched to placebo: 14% weight regain. 89.5% of participants continuing treatment maintained at least 80% of their initial weight loss. Only 16.6% of the placebo group maintained that same threshold. Here’s what people overlook. At week 88, the placebo group was still approximately 9.9% below its original weight. So no, everyone didn’t simply regain everything. But substantial weight regain was common. What the trial didn’t establish: Whether gradual tapering reduces rebound weight gain. How resistance training and dietary interventions might influence weight maintenance. How much of the regained weight consisted of fat versus lean tissue. The takeaway? Tirzepatide isn’t failing when weight returns after discontinuation. Obesity is a chronic condition, and the biological mechanisms driving appetite and weight regulation don’t necessarily disappear because someone lost weight. These medications may be better understood as long-term metabolic therapies rather than temporary weight-loss interventions. That’s an important distinction when discussing expectations, treatment duration, and long-term outcomes. #Tirzepatide #GLP1 #MetabolicHealth #WeightLoss #ObesityResearch #Biohacking #ClinicalResearch

Anesthesiologist here. The patients who bounce back fastest after surgery usually have one thing in common: muscle. Strength after 50 isn't vanity. It's your reserve for the day you need it. Start building it now.

Blanket statements like this are what causes so much confusion in beginning fitness students. Athletes, metabolically healthy people respond to sugar differently. Does this include fruit? Stop with the “Golden Hammer” advice.
Eliminating sugar is the best thing you can do for your body

After 50, the goal isn't just losing weight. It's keeping the muscle that keeps you independent.

Advice for 25-year-olds doesn’t help you stay independent at 80. Neither does “just take it easy.” Coach and I started Doc & Coach because we still train and we still see what works after 50. Strength first.

The most compelling reason I work out to build muscle has very little to do with how I look shirtless. It’s being able to carry your own groceries, get up off the floor, and keep doing the things you love decades from now. Independence is 100% WORTH training for.

I always say we are perhaps living in the single greatest moment in human history, and believe me I stand by it. Here's the proof:  - All-time high literacy rates - Record healthspans  - Energy access globally is at record highs  - Child survival rates have shattered expectations  - Intelligence & energy are collapsing in cost for the first time ever  What else can we ask for?
