According to psychology, the urge to immediately fix a partner’s or friend’s distress instead of just sitting with them in it isn't empathy; it is your own low frustration tolerance. When someone you love is hurting, and you instantly jump into problem-solving mode, offering unsolicited advice or trying to find a silver lining, you are often trying to soothe your own secondary anxiety. You aren't rescuing them from their pain; you are rescuing yourself from the discomfort of witnessing a vulnerability you don't know how to hold.

Cuff mediated airway alignement 🎥👇🏼 Paediatric nasal intubation, tube 4.5 RAE. McGrath VL. Tube impingement on arytenoids, easily fixed by airway alignment. An “old airway trick revisited” Cuff-mediated airway alignment is an advanced intubation technique where the cuff of a tracheal tube (TT) is temporarily inflated to mechanically adjust and center the tube tip with the glottic opening. This maneuver is primarily utilized during difficult intubations—such as when utilizing a videolaryngoscope in patients with restricted mouth opening or an anteriorly positioned larynx—to lift the tube off the posterior pharyngeal wall and direct it smoothly into the trachea. How the Procedure Works: The technique relies on a structured, step-by-step sequence to prevent tissue trauma: [1] Initial Advancement: The clinician advances the TT into the oropharynx using visual guidance from a device like the C-MAC Videolaryngoscope. [1, 2, 3] Cuff Inflation: If the tube tip hinges on local anatomy or misaligns with the glottis, air is gradually introduced into the cuff. Alignment: As the cuff expands against the surrounding pharyngeal structures, it physically pushes and centers the tube shaft, aligning the TT tip directly with the laryngeal inlet. Deflation and Passage: Once optimal alignment is achieved, the cuff is completely deflated so the tip can safely pass through the vocal cords without causing mucosal tearing or laryngeal injury. Final Positioning: The tube is advanced to its proper tracheal depth, and the cuff is re-inflated to establish a definitive ventilatory seal. Clinical Advantages: Navigates Difficult Anatomy: It overcomes "extreme anterior" laryngeal positioning where standard stylets or maneuvers fail to center the tube. Reduces Posterior Hinging: It stops the ETT from getting trapped or scraping against the posterior pharyngeal wall during nasotracheal or videolaryngoscopic intubations. Disadvantages & Safety Risks: Visual Obstruction: Overinflating the cuff within the upper airway can expand the balloon so wide that it completely blocks the clinician's view of the TT by the uvula or cuff. Blind Advancement Risk: If the clinician attempts to push the tube forward before deflating the cuff, it can cause severe friction, mucosal trauma, or nerve compression within the laryngeal space. Some fortunate ICUs have that one aide who seems to know everything: where every piece of equipment is kept, from gauze to 9 Fr catheters to pacing wires; what needs to be reordered; and, just as important, when something should not be reordered at all! They know how to move patients safely, how to calm someone who is frightened or agitated, and how to recognize the subtle signs that a patient is starting to go downhill. They also have a way of making nurses and physicians feel steadier when the unit is under pressure. When they are out sick or on vacation, everyone feels the difference. We are lucky to have this aide as part of our ICU team:

As shown in Panel A, reversal management depends on the urgency of surgery or the invasive procedure. Reversal management includes administration of oral or intravenous (IV) vitamin K with or without 4F-PCC, depending on the timing of the procedure (emergency or urgent), baseline international normalized ratio (INR) value, and presence (or absence) of active bleeding. For patients receiving direct oral anticoagulants (DOACs), the decision also depends on time to surgery. Decision making is informed by DOAC type, time since last dose, half-life, presence (or absence) of active bleeding, and renal function tests to estimate residual drug activity. Panel B shows reversal strategies for patients presenting with major bleeding while receiving an oral anticoagulant. The reversal strategy of vitamin K antagonists includes vitamin K given intravenously or orally, combined with 4F-PCC and INR testing. Management of anticoagulant reversal of direct oral FXaIs is based on four key factors (shown as the 4Ts): type of bleeding, timing of the last dose, thrombotic risk, and need for invasive procedures in the next 48 hours that would result in the administration of UFH. These factors may facilitate the use of specific (e.g., andexanet alfa) or nonspecific (e.g., 4F-PCC) antidotes. The reversal of dabigatran is informed by three clinical variables (shown as the 3Rs) — the type of bleeding, time of the last dose of dabigatran, and preserved renal function. This standing ovation totally brought me to tears. On Thursday, my dad (stage 4 pancreatic cancer) started a Revolution Medicine trial building upon these results — RMC 5127, which uses the drug celebrated here, Daraxonrasib, with another drug targeting his specific KRAS mutation, G12V. He’s nearly 2 years into his fight with pancreatic cancer and we have personally felt the impact of this watershed moment. It is a lifeline. To the incredible, stalwart, brilliant researchers and folks bringing these discoveries to life - thank you. Thank you thank you thank you.
One of the most amazing things I’ve ever seen: a standing ovation for the full Daraxonrasib results I feel inspired and energised, to put it mildly — we have a targeted therapy for pancreatic cancer now, and nothing is undruggable anymore

This is not simply a new pancreatic cancer drug. It is a reminder that even “undruggable” biology can become treatable with persistence. Daraxonrasib doubled median OS vs chemotherapy in RAS G12 metastatic pancreatic cancer: 13.2 vs 6.6 months. A remarkable ASCO moment. #ASCO26 @DrChoueiri @TiansterZhang @CathyEngMD @montypal @tompowles1 @brian_rini @cdanicas @GlopesMd @PGrivasMDPhD @nataliagandur @yekeduz_emre @neerajaiims @ASCO @ONCOassist @OpenMedicineHQ @MedwatchKate @scserendipity1 @CParkMD @urotoday @OncLive @crisbergerot @urologysummit @SuyogCancer @Larvol @IMG_Oncologists
