◾️In memory of Prof L Gattinoni, towering figure in ICM, profoundly shaping understanding of #ARDS & #ECMO. The “father" of prone position revolutionized MV with a pioneering "baby lung” concept. His legacy will guide/inspire for generations to come.

Awake cannulation in VV #ECMO: uncommon but has recently increased
🔍 @ELSOOrg Registry: > 28.6K adults, 2016-2022
🔎 2.8% remained spontaneously breathing during cannulation
🏥 similar survival vs patients cannulated on MV
Is associated with shorter LoS & increased #ICUrehab in patients considered for lung #transplantation.

#ECMO for tuberculosis-related #ARDS?
🫁 Feasible! International study reporting 51% survival. Better outcomes for miliary vs cavitary TB. Age, drug-resistant TB, pre-ECMO SOFA score indicators of poor prognosis, to be considered as assessing candidacy

Anticoagulation modalities & bleeding events in adults on VV #ECMO, insights from PROTECMO, large international study, illustrating actual daily practice:
💉UFH main anticoagulant used, targeted on aPTT usually between 40-60’'
🩸bleeding events occurred in about 50% of patients, fatal in only about 1% of cases
🧪 higher aPTT major determinant for first bleeding episode
Reducing targets may be effective in minimizing risk of hemorrhages?? @ATSBlueEditor

🚧 Pushing the limits initiating #ECMO in immunocompromized patients?? @Mat_DiNardo @bambinogesu Hospital at #APELSO2023
#ECLS in not for all the patients, but we should decide on a case-by-case basis, also considering potential for recovery and available therapies (involve oncologists in discussion!): if the patient is deemed a good candidate, full support including ECMO and other extracorporeal strategies as #RRT should be implemented in the setting of a time limited trial approach.
