📢 Now published: The Saudi Heart Association Position Statement on the Management of Advanced Heart Failure in Saudi Arabia — developed by the Saudi Heart Failure Working Group. 🇸🇦🫀
An estimated 32,000–55,000 people in the Kingdom live with advanced HF. Yet referral remains late, and access uneven — only 3 transplant-capable centers, all in Riyadh.
Our consensus provides a national framework:
✅ Clear definition & warning signs of advanced HF
✅ INTERMACS & SCAI-based prognostic stratification
✅ Structured referral pathways (hub-and-spoke)
✅ GDMT optimization — without delaying referral
✅ Early palliative care integration
✅ A proposed national registry with defined KPIs
Recognize early. Refer on time.

Great discussion today on inflammation in HFpEF/HFmrEF with Dr. Lam.
My take home: HFpEF/HFmrEF are systemic syndromes where comorbidities drive chronic inflammation, endothelial dysfunction, microvascular inflammation, fibrosis, and stiffness.
Dr. Lam highlighted the clinical opportunity: hsCRP ≥2 mg/L can help identify inflammatory risk, and POSEIDON showed ~2/5 patients with HF have high inflammatory risk supporting future anti-inflammatory strategies across the HF spectrum.
