8 years ago today, I began an unexpected journey from SICK to RECOVERING. I ain’t done. Today, I reflect on my struggle from the #ICU, to my new life. Grateful for this milestone & praying other EtOH’s also find Doctors & Nurses who don’t give up on them!🥰 #Walk4PICS #MedX #LOVE

If you deliberately ignore #delirium you will never be a good doctor.

SNAP-3: 7,128 UK patients aged 60 and over having surgery in 214 hospitals. Median hospital stay was 12.5 days with delirium and 1 day without.
After adjusting for age, frailty, multimorbidity and type of surgery, delirium added 5.2 days (95% CI 3.8 to 6.5), and the odds of death within a year doubled. Observational, so some influences were not measured.

It was a privilege & a pleasure. Thank you @ANZDA_delirium
10 years on from critical illness, the long term impacts (sequelae 😉) of delirium are obvious. & I’m very fortunate symptoms not worse. I am SO grateful for my care & yet it was not without cost. LOVE ICU CLINICIANS

Delirium care crosses professional boundaries.
Medical treatment, nursing care, therapy, pharmacy and family knowledge all contribute.

I'm sick of the collective complacency and mediocrity around delirium care.
It's like we all give each other permission to just be collectively crap at it.
Low standards that we would never accept for any other emergency condition.

Mrs A, 82, was admitted "off legs and confused". A urine dip was positive, a diagnosis of "urosepsis" was written, antibiotics started, and everyone relaxed.
She did not improve. On day three someone finally examined properly: she was dry as a bone, and her chest crackled at the right base. The urine had told us nothing except that she was old and had bacteria in her bladder, as many older women do without it meaning anything.
Rehydration and the right antibiotic for her pneumonia, and her mind came back.
A positive dip in an older person with delirium is not a diagnosis. Asymptomatic bacteriuria is common, and treating it while the real cause hides is one of the easiest mistakes in geriatric medicine. Examine the whole patient before you blame the bladder. #delirium
