Manchester City have been found guilty on almost all of the charges relating to breaches of the Premier League’s financial regulations.
The sanctions have not been decided, with all possibilities remaining on the table, and City are expected to appeal.

It's making sure it never comes back.
That is why en bloc resection is paramount.

🔑 Piecemeal resection occurred in 11.2% of cases
🔑 Piecemeal resection was associated with higher recurrence
🔑 Every additional 1 mm in lesion size increased the likelihood of piecemeal resection
🔑 Flat lesions were also more likely to require piecemeal resection
The best strategy for recurrence begins before the first cut.
Choose the technique most likely to achieve en bloc resection.

No es DaVinci, es la plataforma de Quinta Generación KangDuo SR2000 de Sagebot. Es el primero en Ecuador, el segundo en Latam en el momento de la Cirugía y el primero en el mundo occidental en realizar un procedimiento en un humano.
Actualmente, la mayoría de compañías de seguros brinda la cobertura para cirugía robótica.
Veo que están usando un da Vinci de @IntuitiveSurg el cual es uno de los modelos más avanzados y ampliamente utilizados en todo el mundo. En Ecuador hay varios, y hasta la última vez que me informé el @IESSec contaba con estos equipos en Quito y Guayaquil.
El primero hospital privado en tenerlo fue Metropolitano de Quito. Entiendo que en Guayaquil también lo tiene @SolcaGuayaquil
Ojalá se les de más uso pues son equipos con alto costo de adquisición y mantenimiento.

Is it Barrett’s? Or just "Burnt Salmon"? 🍣
We are taught to look for "salmon-colored" mucosa. But as this slide from EndoCollab shows, there are 50+ shades of salmon.
The Clinical Reality: Identifying the color change is only Step 1.
To confirm the diagnosis, you must map the landmarks:
1️⃣ Locate the Gastric Folds (the true GEJ).
2️⃣ Look for the palisade veins (which exist only in the esophagus).
3️⃣ If the "salmon" color extends above these landmarks, you are in the danger zone.
Don't rely on color alone. Trust your anatomy.

Esophagus is 25 mm × 25 cm tube.
Diameter:
•16–18 mm→strictures/anastomoses, but ↑migration
•23 mm→leaks, fistulas, strictures; ↑discomfort, perforation
Length: 10, 12, 15 cm
•Extend ≥2 cm beyond stricture
•GEJ involved: Long stents

What can go wrong when tattooing the colon?
I was always focused on not injecting it too aggressively.
This one seemed neat, but I went too “supra-submucosally,” as a few weeks later during surgery there was no tattoo.
As everywhere: the best path is in the middle.
